AFFIDAVIT OF CONSENT
I, ROBERTO B. VICENTE of legal age, Filipino and resident of Barangay
Hangyad, Bais City, Negros Oriental, Philippines, after having been sworn to in
accordance with law do hereby depose and state:

I am the heir of GASPAR VICENTE who was registered owner of a parcel

of land, Ass. Lot No. 010 with Tax Declaration No. 2018-21-052 located at
Barangay La Paz, Bais City ,Negros Oriental.

That I hereby give consent to YOLANDA C. OLORES to connect electrical

installation within my residence which is situated in a portion of the above-
mentioned lot;

I am executing this affidavit of consent upon the request of YOLANDA C.

OLORES in compliance with the requirements of NORECO I, for her application
for
electrical installation.
