AFFIDAVIT OF CHANGE COLOR

I, RURAL TRANSIT MIN INC., of legal age, and with address at RTMI CMPD BULUA HI-WAY – CAGAYAN
DE ORO CITY (Capital) MISAMIS ORIENTAL REGION 10, after having been duly sworn according to law, hereby depose
and say that:

1. I am the (CORPORATE SECRETARY OF DAVAO SOLAR BEST CORPORATION WITH BUSINESS AT R. CASTILLO ST.,
AGDAO, DAVAO CITY);

2. The corporation is the registered owner of a certain motor vehicle that is more particularly described as follows:

Make: HINO Engine No.: J08CTK12242

Type: TRUCK BUS Chassis No.: RK1JMT10474
Color: RED/WHITE Plate No.: 1008-80559

3. The original color of the said motor vehicle was “red/white”;

4. When the paint of the above-described motor vehicle had faded, we had its color repainted to “ blue/white” such that
the color of the said motor vehicle was changed from “red/white” to “blue/white” ;

5. Said change color is not intended for any illegal of unlawful purpose but solely due to the foregoing reasons;

6. I am executing this Affidavit to attest to the truth of the foregoing and in support of the application for the change of
color of the above-described motor vehicle on its Certificate of Registration.
