REITERATION OF DEPED TASK FORCE COVID 19 MEMO NO. 575:

IATF VACCINATION REQUIREMENT FOR PERSONNEL
REPORTING ONSITE

PUBLIC SCHOOL DISTRICT SUPERVISORS
SCHOOL HEADS
TEACHING AND NON-TEACHING PERSONNEL

1. This is to inform everyone that this Office requires all personnel to

present their vaccination card upon entering in the office/school
premises, effective January LA, 2022.

2. For those unvaccinated personnel reporting onsite they shall be

required to present a negative result RT-PCR Test undertaken at most 48
hours before the day of reporting, and which shall be valid for up to two
weeks from the day of testing, unless the personnel develop symptoms,
for which they shall stop reporting on-site and proceed to established
health protocols. Failure to do so, they are considered absent.
The RT-PCR or antigen test of unvaccinated personnel who have
refused to be vaccinated despite being eligible for vaccination shall be
charged at the personnel's expense.
3. Parents are also required to present their vaccination card when
entering the school premises to get the modules.

4. For those unvaccinated parents they shall be entertained outside of

the school gate.
