1.0 Purpose

This standard defines Saxon Energy Services Inc., its subsidiaries, and affiliates (collectively,
“Saxon”) standards and expectations relating to the effective management of health, safety,
or environment (HSE) Events in order to provide appropriate care for injured or ill Saxon
Employees and minimize negative impact to the organization. This standard includes
requirements pertaining to HSE Event reporting, investigation, root cause analysis,
corrective/follow‐up action, and compliance monitoring.

2.0 Scope

This standard is applicable to all Saxon operations worldwide, and is recommended for any joint
ventures where Saxon manages or co‐manages the operations.

Historically, Saxon used two QHSE Software Applications for HSE Event reporting and management:
QUEST and SIIMS. As of Q4 2013, QUEST became Saxon’s single, global application.

3.0 Definitions

TERM DEFINITION
Accident One or more related HSE Incidents that have resulted in
HSE loss(es). For example:
• Harm to people (e.g., fatality, injury, or
illness)
• Damage or loss to property (e.g., vehicles,
facilities)
• Damage to the environment (e.g., pollution,
contamination, etc.)
• Damage to, or loss of reputation
• Losses of confidential data
See Appendix E: HSE Severity Matrix below for incident
severity classifications.
Combined Lost Time Incident The number of total Fatalities, Lost Work Day Cases,
Frequency (CLTIF) and Restricted Work Day Cases per 1,000,000 manhours
worked.
CLTIF = (CLTIs/manhours worked in month) X 1,000,000
Contractor An individual or organization performing work for
Saxon, following verbal or written agreement.
‘Sub‐contractor’ is synonymous with ‘Contractor’.
(Contractor may include suppliers of products, but
does not include courier services, restaurant delivery, or

A rig moving company directly contracted by Saxon is
considered a Contractor.
A rig moving company not directly contracted by Saxon
is considered a Third Party, not a Contractor.
Contractor Employee A person employed by a Contractor or Contractor’s
Sub‐contractor(s) that is directly involved in execution of
prescribed work under a contract with Saxon.
Disability Any physical or mental impairment that limits activity.
A Disability may be partial or complete, temporary or
permanent.
Fatality Accident Rate (FAR) The number of work‐related fatalities per 100,000,000
(100 million) manhours worked.
FAR = (Fatalities/manhours worked in month) X

First Aid Case Cases that are not sufficiently severe to be reported
as medical treatment or more severe cases, but
nevertheless require minor first aid treatment (e.g.,
dressing on a minor cut, removal of a splinter from a
finger). First aid cases are not Recordable.
Following are some examples:
• Use of OTC (over‐the‐counter) medication at
non‐prescription strength
• Tetanus immunization
• Cleaning, flushing or soaking wounds on the
skin surface
• Bandages (e.g., Band‐Aids, gauze pads, steri‐
strips or butterfly bandages)
• Hot or cold therapy
• Non‐rigid support‐wraps, elastic bandages, etc.
• Temporary immobilization devices for transport
• Drilling fingernail or toenail or draining blisters
• Using eye patches
• Simple irrigation or a cotton swab to remove
foreign objects from the eye that are NOT
embedded
• Tweezers or other simple means of splinter or
foreign material removal (non‐eye)
• Finger guards
• Massages
• Drinking fluids to relieve heat stresses

NOTE: Any other treatment makes the HSE Event
recordable.
Hazard Any object/body/effect, substandard act, or substandard
condition that has potential to cause an HSE Event.
Hazardous Situation (HS) An unplanned or undesired situation in which the Risks
created by an activity’s exposure to a Hazard(s) are
inadequately controlled leading to the potential to cause
an Accident or Near Accident.
Heavy Vehicle Any motor vehicle having a GVWR (Gross Vehicle
Weight Rating) or GCWR (Gross Combination Weight
Rating) greater than or equal to 10,001 pounds (4500
kgs), unless otherwise determined by local laws or
regulations.
High Potential (HiPo) Event Any HSE Event with Potential Risk defined as ‘High’ (Red)
in the Saxon Operations Risk Matrix.
Hospitalization Formal admission as a patient into a medical facility.
HSE Event An Accident, Near Accident, or Hazardous Situation.
HSE Event Report/QUEST Risk First report of an HSE Event generated by QUEST after
Identification Report (RIR) initial required information is populated on main
screens.
HSE Incident Any distinct event that results in or has potential for HSE
loss(es), including personal injury, environmental,
security, driving, property damage, etc. This includes
Accidents and Near Accidents but excludes Hazardous
Situations.
Intensive Care Admission of a patient in the intensive care unit (ICU) of a
hospital for any length of time. Continuous and closely
monitored health care provided to critically injured or ill
patients, generally in a specialized section of a hospital
containing the equipment, monitoring devices, and
medical and nursing staff to provide such care.
Light Vehicle Any motor vehicle having a GVWR (Gross Vehicle
Weight Rating) or GCWR (Gross Combination Weight
Rating) less than 10,001 pounds (4500 kgs), unless
otherwise determined by local laws and regulations.
Likelihood A measure derived from the combination of frequency
and probability for an Incident to occur.

Lost Time Incidents (LTI) The number of injuries or illnesses to Saxon and/or
Contractor Employees that have a Lost Work Day
Case. The number of LTIs is the sum of fatalities and Lost
Work Day Cases.
Lost Time Incident Rate (LTIR) The number of Lost Time Incidents per 200,000
manhours worked.
Lost Work Day Case (LWDC) Any injury or illness associated with a work‐related
(or Saxon‐related) activity, other than a fatal injury or
illness, that results in a person being unfit for work on
any day after the day of occurrence of the Occupational
Injury or Illness.
‘Any day’ includes rest days, weekend days, leave days,
public holidays, or days after ceasing employment.
(Logistical delay for the purpose of medical examination
shall not be taken into account when establishing the 24
hour period.)
Lost Work Day Case Rate The number of Lost Work Day Cases per 200,000
(LWDCR) manhours worked.
LWDCR = (LWDCs/manhours worked in month) X 200,000
Manager A Saxon Employee who has at least one or more Saxon
Employee(s) reporting to him/her. Generally, this
includes positions such as Driller, Rig Manager, Rig
Superintendent, Operations Manager, Country Manager,
Region VP/Director as well as staff positions with similar
responsibilities.
Medical Treatment Only Case Medical Treatment includes managing and caring for a
(MTO) patient for the purposes of recovering from an injury or
illness. All Medical Treatment Only Cases are recordable
with few exceptions such as the following:
• Visits to a doctor or health care professional
solely for observation or counseling
• Diagnostic procedures, including administering
prescription medications that are used solely for
diagnostic purposes
• Any procedure that can be labeled first aid
A Medical Treatment Only Case is an Accident other
than a Fatality, Lost Work Day Case, Restricted Work Day
Case, or a First Aid Case.
Motor Vehicle Crash Any Saxon Work‐related vehicle damage or personal
injury due to a vehicle‐related Accident while driving a

Saxon Vehicle or personal or rented vehicle while on
Saxon business. See Appendix E: HSE Severity Matrix for
vehicle‐related Accident severity classifications.
Motor Vehicle Crash Rate CMS The number of Saxon Work‐related Catastrophic,
(MVCRk CMS) or Automotive Major, and Serious (CMS) automotive crashes per
Accident Rate (AAR)k CMS million kilometers driven.
Motor Vehicle Crash Rate CMSL The number of Saxon Work‐related Catastrophic,
(MVCRk CMSL) or Automotive Major, Serious, and Light (CMSL) automotive crashes
Accident Rate (AAR)k CMSL per million kilometers driven.
Near Accident (NA) One or more related events that under slightly different
circumstances could have resulted in HSE loss(es). This is
generally called a ‘Near Miss’ in the drilling industry.
Near Miss For the purposes of event reporting in Saxon, a Near Miss
is only used for Service Quality Events as defined in the
Service Quality (SQ) Event Reporting and Management
Standard (OPS.COR.STN.001.EN) and used in QUEST.
Non‐Occupational Illness An illness or medical condition caused by a person’s
health condition not related to the working
environment or a Saxon process.
Non Saxon‐related Activities Inclusive of all other Non Saxon‐related activities.
Concerning All Other Third All other HSE Events are classified as Saxon Non‐
Parties Involved/Non Concerned Events.
Non Saxon‐related Activities Inclusive of activities:
Concerning Employees, • During vacation or days‐off
Dependents, Customers, and • As a result of a self‐inflicted injury or illness
Contractors such as suicide or drug overdose
• As a result of a Non‐Occupational Injury or Illness
• As a result of sporting and/or recreational
events (including Saxon sponsored) regardless of
location and where management controls are
not expected
• Outside working hours when not in Saxon
controlled facilities
• Commuting to work
HSE Events occurring during Non Saxon‐related activities
are classified as Saxon Non‐Involved/Concerned events.
Non Work‐related Activity Other situations defined in OSHA 1904.5(B) as detailed
in Appendix G: Work Relatedness, such as eating or

drinking in the work place, and personal tasks are
potentially considered non work‐related.
Number of Fatalities The number of Saxon Employees and Contractors that
died as a result of an HSE Event (associated with a
Saxon‐ related activity). ‘Delayed’ deaths that occur
after the Incident are to be included if the deaths were a
direct result of the Incident.
Number of Lost Work Days The total calendar days, consecutive or otherwise,
after the days on which the Occupational Injuries or
Illnesses occurred, where persons reported under LWDC
above were unfit for work and did not work.
Occupational Illness Any abnormal medical condition or disorder of an
employee, other than one resulting from an
Occupational Injury, caused by exposure to
environmental factors associated with employment
(OSHA, 1986) and which results in death; being unfit to
work the day following the event; restriction of work or
motion including temporary or permanent transfer to
another job. The environmental factors associated with
employment include exposure to chemical, physical,
biological, and/or ergonomic factors.
Occupational Injury A work‐related physical injury that results in death;
being unfit to work the day following the event; or
restriction of work or motion including temporary or
permanent transfer to another job.
Permanent Disability An injury that impairs the physical and/or mental ability
of a person to perform their normal work or non-
occupational activities supposedly for the remainder of
their life. Generally under workers' compensation laws
covering occupational injuries, once the condition is
stable, a degree of Permanent Disability is established
even if the person is able to work despite the physical
problem. For example, an amputation or partial
amputation of a finger is a Permanent Disability that may
or may not prevent an employee from returning to work
on a Saxon rig.
Potential Risk Risk Level (as defined in the Saxon Operations Risk
Matrix) before the implementation of risk control
measures.
Recording Restricted Work/ The number of calendar days when the employee was

Transfer/Days Away from Work restricted, transferred, or away from work.
• This includes all calendar days, without
exception and includes days‐off, weekends, and
holidays
• The number can be estimated at first and
finalized when correct information is known
• Maximum of 180 days (so‐called “cap”), even if
it carries over to the following year (only enter
the Accident once)
• For a fatality, the number is 180 days
Remedial Work Plan (RWP) A listing of action items created in QUEST.
Residual Risk Risk Level (as defined in the Saxon Operations Risk
Matrix) after the implementation of risk control
measures.
Restricted Work Day Case The total number of injuries or illnesses to Saxon
(RWDC) Employees and/or Contractor employees, other than a
Fatality or Lost Work Day Case that results in a
person being unfit for full performance of the regular job
on any day after the Occupational Injury or Illness.
Risk A measure of the Likelihood of an HSE Event occurring
with a negative outcome in terms of personal injury,
illness, environmental impact, or damage to property or
equipment in combination with the Severity.
Risk Level Five levels of Risk defined in the Saxon Operations Risk
Matrix: Insignificant (Blue), Low (Green), Medium
(Yellow), High (Red), or Extreme (Black).

Saxon‐related Activity Inclusive of activities:
• Taking place at any Saxon controlled facility
while off‐duty
• Involving a Saxon Vehicle (see definition in
Section 3.0) or a Saxon asset outside of a work‐
related activity
• Involving any private vehicle or taxi on Saxon
business not involving a Saxon employee
• Involving any customer or Contractor
(professional or social) activity related to
Saxon
• Involving the use or access to Saxon computers
or networks
• That is Saxon organized (except for any
participation in physical sports) and taking
place off site where management control is
expected
HSE Events occurring during Saxon‐related activities are
classified as Saxon Involved/Non‐Industry Recognized
events.
Saxon Driver A person that drives a Saxon‐owned, leased, rented,
or personal vehicle as part of their duties. This excludes
Saxon Employees commuting to their regular place of
work, unless specifically included by the regulations of
the country in question.
Saxon Employee Any person employed by and on the payroll of Saxon,
including corporate and management personnel.
Persons employed under short‐service contracts
(more than 90 days) are included as Saxon Employees
provided they are paid directly by Saxon.
Saxon Operations Risk Matrix A two-dimensional matrix used in Saxon operations to
quantify Risk Levels by plotting Likelihood versus
Severity.
Saxon Vehicle Any motor vehicle owned, leased, rented, or
contracted by Saxon. Exceptions to this definition for the
applicability of these requirements are only the vehicles
that are assessed as lower Risk by the Saxon QHSE
Department and are used in a Saxon yard, warehouse,
or on the rig site, and may include forklifts, mobile yard

Saxon Work‐related Activity An activity in a work environment or in the course/scope
of work duties, that is or ought to be subject to
management controls.
Injuries or illnesses incurred on Saxon or Contractor
premises while off‐duty are not classified as Occupational
Injuries or Illnesses and should not be reported as Saxon
Work‐related Activities since they are not work‐related,
but they will be reported in accordance with Section 6.0
of this document as applicable. The exception to this rule
is when the injury or illness owes directly to the activities
of other personnel who are at work or within the scope
of work duties.
The commute between an employee’s regular place of
work and home is not considered work‐related.
Additionally, HSE Events will be considered Saxon Work‐
related Activity if an event or exposure in the work
environment:
• Caused the injury or illness
• Contributed to the existing condition
• Significantly aggravated a pre‐existing injury or
illness (must result solely from a non‐work‐
related event
HSE Events occurring during work‐related activities are
classified as Saxon Involved/Industry Recognized events.
Severity Four measures of loss consequences resulting from an
Incident: Catastrophic (C), Major (M), Serious (S), or
Light (L).
Third Party Anyone who is not a Saxon Employee, a Contractor
Employee, or a customer.
Total Recordable Incidents (TRI) The total number of Fatalities, Lost Work Day Cases,
Restricted Work Day Cases, and Medical Treatment
Only Cases.
Total Recordable Incident The number of recordable Incidents per 1,000,000
Frequency (TRIF) manhours worked.
TRIF = (TRIs/manhours worked in month) X 1,000,000
Total Recordable Incident Rate The number of recordable Incidents per 200,000
(TRIR) manhours worked.

cost of an Accident.

4.0 Responsibilities

4.1 Managers
Managers are responsible for compliance with this standard, for leading and sustaining
the HSE culture, and for being committed to HSE excellence. In particular, managers
shall ensure that:
• All HSE Events are reported in QUEST and are notified to higher management
via telephone (See Section 9.1.3 and Appendix D: HSE Event Notification
Protocol via Telephone) according to this standard
• A reporting culture is developed including the proactive reporting of all HSE
Events and Suggestions by all personnel
• Timely and thorough investigations and root cause analysis are conducted by
the appropriate level of management in accordance with the responsibility
matrix in Appendix A: HSE Event Reporting and Management Responsibility
Matrix
• A Root Cause Analysis shall be conducted on all HiPo HSE Events (Accidents,
Near Accidents, or Hazardous Situations)
• Appropriate remedial work plans (RWPs) are developed, implemented,
followed‐up, and reviewed with Country/Region management and closed out
within 90 days of HSE Event report creation
• HSE Event reporting is verified: HSE Event description, Severity, Potential Risk,
investigation, RWP closures, Residual Risk, etc.
• All appropriate management levels shall be responsible to report HSE Events in
QUEST and shall subscribe to HSE Event notification reports. At a minimum,
such Subscriptions shall be configured per the responsibility matrix in Appendix

improvement with Operations
• Assist managers in conducting HSE Event investigations
• Ensure any delegation of responsibilities in Appendix A: HSE Event Reporting
and Management Responsibility Matrix shall be only to qualified personnel

5.0 Process

Saxon is committed to loss prevention. If, however, an Accident or release does occur, the
potential for recurrence must be minimized and/or prevented. All Accidents must be reported,
assessed, investigated, and analyzed to prevent their recurrence.
5.1 HSE Event Reporting and Management Tool and KPIs
QUEST shall be the standard Saxon electronic systems for reporting all HSE Events,
Suggestions, Audits and Inspection reports, Remedial Actions, Meetings, Recognitions,
and Exemptions.

The following should be used as guidance when generating QUEST reports:


• The report must describe the risk exposure
• The report description must be objective, clear, and complete
• The report must never be offensive
• Although names may be used for the Eight Minimum Facts described in section
9.1.4, names and other personal/confidential information should NOT be used
in the Description section of QUEST. Personal/confidential information
contained in attachments should be marked out before uploading the document
QUEST
• QUEST is NOT to be used to vent frustration
• All CMS (Catastrophic, Major, and Serious) Accidents must be reported in
English

Remember, what is recorded in QUEST:


• Can be read by anybody with access to the application
• Will be read and acted upon by the appropriate level of management
• May be subject to disclosure as part of legal proceedings

Data for each QUEST location shall be entered monthly into QUEST’s SafetyNet function
by the 5th of the month. Mandatory data elements are listed in the QUEST section of
Appendix B: QUEST Mandatory Monthly Input Definitions.

HSE Key Performance Indicators (KPIs) allow management to quantify and monitor HSE
performance in Saxon. These indicators are defined in Appendix C: HSE Key Performance
Indicators (KPIs) ‐ Saxon Involved/Industry Recognized.
5.2 HSE Event Activity Type Classification
HSE Event activity type classification is based on the nature of the activity being
performed at the time of occurrence of the HSE Event.

The following four possibly activity types are defined in Section 3.0:
• Saxon work‐related Activity
• Saxon‐related Activity
• Non‐Saxon related activities concerning employees, dependents, customers,
and contractors
• Non‐Saxon related activities concerning all other third parties
5.3 Contractor and Third Party‐related HSE Events and KPIs
Contractor and Third Party HSE Events shall be reported in QUEST and should be
classified according to the contractual relationship or lack thereof with Saxon.
• All Contractor work‐related HSE Events occurring where the Contractor is
directly contracted by Saxon:
o Shall be considered work‐related activities and reported as “Saxon
Work‐related Activities” in QUEST
• All Contractor work‐related HSE Events occurring where the Contractor is not
directly contracted by Saxon (e.g., express delivery companies, freight
forwarders, rig moving companies not directly contracted by Saxon, etc.)
o Shall be considered non Saxon‐related activities and reported as “Non
Saxon‐related activities concerning employees, dependents, customers
and contractors” in QUEST
5.4 Multiple Losses
HSE Incidents with multiple losses occurring simultaneously shall be reported as one
HSE Incident with each loss reported separately in the same report. For example:
• A blowout is a single HSE Incident that could include the following losses:
multiple injuries, equipment damage, and an environmental spill
• A single Motor Vehicle Crash or rollover could result in the following losses:
several Recordable Injuries and/or Lost Time Injuries and equipment damage
5.5 Severity Classification of Accidents
Work‐related Accidents are classified as Catastrophic, Major, Serious, or Light (CMSL)
according to the severity of the resulting injury, illness, or loss based on the sum of all

known resulting direct and indirect costs regardless of who is responsible or required to
pay these costs (irrespective of any possible insurance recovery and/or actual legal
liability). The criteria for each of the Severity levels is defined in Appendix E: HSE
Severity Matrix.

The following rule applies to the reporting of all Accidents: in cases where more than
one Severity category applies, the most stringent criteria must be assumed for the
entire Accident. For example, an Accident involving a Light injury to a Saxon Employee
and a Serious environmental loss must be classified as a Serious Accident, not a Light.
5.6 Risk Classification of HSE Events
Risk is a measure of the Likelihood of an HSE Event occurring with a negative outcome in
terms of personal injury, illness, environmental impact, or damage to property or
equipment in combination with the Severity.

Both Potential Risk (Risk Level before the implementation of risk control measures) and
Residual Risk (Risk Level after the implementation of risk control measures) are derived
in QUEST based on answers provided by the employee creating the RIR. It is the
responsibility of the RIR creator to define the Potential Risk and Residual Risk. The
employee who closes the report in accordance with Appendix A is responsible to ensure
that the risks assigned are reasonable.

Process flows for classifying both Potential Risk (QHSE.COR.REF.029.EN) and Residual
Risk (QHSE.COR.REF.030.EN) can be found in QUEST and MySaxon’s The Source.

The Saxon Operations Risk Matrix is defined in Appendix F: Saxon Operations Risk
Matrix.
5.7 No Fault Reporting
Saxon has a system of no fault reporting for all HSE Events that occur during the course
of work. No fault reporting of HSE Events enables Saxon Employees to recognize
management's commitment to improving HSE performance by ensuring they can report
all undesired and unplanned events and Hazardous Situations without fear of reprisal.
Management shall analyze the events that took place leading up to HSE Events and
identify the root cause of the occurrence. However, no fault reporting of HSE Events
neither alleviates responsibility for safe behavior nor conflicts with Saxon’s
accountability guidelines.
5.8 Initial Response to HSE Incidents
HSE Incident management procedures should be initiated only after responding to HSE
Incidents by:
• Ensuring, as appropriate, the safety of personnel and any members of the public
• Initiating Emergency Response Plans, if necessary
• Tending to the needs of the injured

5.9 Internal Reporting
As a general principle, most HSE Events (i.e., Accidents, Near Accidents, and Hazardous
Situations with CMS potential) must be REPORTED IMMEDIATELY to the appropriate
Saxon Supervisor. In addition, regardless of how minor, HSE Events must also be
reported in QUEST. All required information in the initial reporting screens must be
input in order to prepare an initial report.

HSE Events shall be notified through the chain of command, starting with the direct
supervisor, by telephone (or in‐person whenever possible) according to the table below
and Appendix D: HSE Event Notification Protocol via Telephone.

NOTE: Telephone notification in the QHSE function shall be in line with the protocol listed above in the
“Top Level Notified” column for Operations.

Any written witness statements and/or other evidence must be kept with the
HSE Event Report and scanned and uploaded into QUEST.

Certain Regulatory Agencies may require that a record be kept at the site
recording information regarding all Accidents. Any record of Accidents, including
First Aid Cases, must be kept on file for a period of at least three years from the
date of last entry or per local Country regulations, whichever timeframe is
greater.
5.10 Eight Minimum Facts
The eight minimum facts below shall be reported for all CMS Accidents and Light
Accidents with potential CMS Severity, Accidents in QUEST in the Detailed Description:
1. Region, Country and name of the location, rig, facility, etc.

2. Brief description of the HSE Event including type

8. Brief description of asset, environmental, information, or reputation loss, if any.
The Reporter should mention similar operations on other Saxon rigs if
applicable.

At this time the person responsible for report entry should also consider if Access
Control is required, consulting with higher management levels if needed. This means
he/she will have to select a group of QUEST users that can view the report. See Section
9.1.6 for further explanation.
5.11 Other Notifications
Legal advisors and Human Resources personnel shall be notified of all Catastrophic
Accidents and any Accidents that may result in legal actions or result in negative
attention from the media (e.g., environmental spills).
5.12 QUEST Report Confidentiality and Access Restrictions
An HSE Event that is deemed to include sensitive information that may imply Saxon
liability shall be treated as confidential in QUEST. QUEST Access Control can be applied
to restrict those people who can view the report. If Access Control is applied to the
QUEST report within 60 minutes after the report is saved, then QUEST notifications (i.e.,
Subscriptions) will only be sent to those persons on the Access Control list.
5.13 External Reports
Coordination of external reporting of any HSE Event is the responsibility of the Country
Manager and Region QHSE Department. Region VP/Director approval is required prior
to external distribution of any reports.
5.14 Government Requirements‐Verbal Reports
Specific reporting requirements shall be detailed in the Country Emergency Response
Plan regarding any required verbal reporting of HSE Events to government agencies.
5.15 Government Requirements‐Written Reports
Specific reporting requirements shall be detailed in Country Emergency Response Plans
regarding any required written reporting of HSE Events to government agencies.

Region VP or Director approval is required prior to external distribution of any written


reports other than to customers or government agencies responsible for Workman’s
Compensation.

All external contact with government agencies related to HSE Event reporting or follow‐
up shall be completed through the QHSE Department.
5.16 Accident/HSE Event Investigation
The following HSE Events shall be investigated to determine root causes per the roles
and responsibilities matrix in Appendix A: HSE Event Reporting and Management
Responsibility Matrix:
1. All Catastrophic, Major, and Serious Accidents
2. All HiPo Light Accidents
3. All other HiPo HSE Events (Near Accidents or Hazardous Situations)

The goal of Accident/HSE Event investigation is to get to the root causes and identify
failures in the Saxon QHSE Management System to ensure that action can be taken to
prevent recurrence. Investigations are not intended to lay blame. The product of
investigations should be constructive information that can be applied to other situations
as proactive preventive measures.

Per Appendix A: HSE Event Reporting and Management Responsibility Matrix, the
investigation team leader may assign specific responsibilities to team members as soon
as possible. Investigation team members should have knowledge of root cause
determination and analysis and should receive SCAT training in HSE Incident
investigation. This training will be made available to all applicable levels of management
by the Saxon QHSE Function.

Investigations shall take place as soon as possible following an HSE Event because delay
may change or alter the perception of the facts, distort evidence, and make it difficult to
determine the immediate or underlying causes. Investigation teams must begin the
process at the site of the HSE Event within 24 hours of the occurrence.
5.17 Preservation of Information Following a CMS Accident
The condition of the Accident site must not be changed or altered other than
what is necessary to protect people from imminent and immediate danger and
stabilize the situation.

Preservation of information should commence once the situation at the


Accident scene is stabilized in terms of safety of personnel, any injured parties
are under appropriate care, and contingency measures are implemented. If
possible, the Rig Manager/Rig Superintendent (RM/RS) should take measures
to ensure the site is not disturbed prior to the investigation team's arrival.

In the case of Accidents, the RM/RS must obtain written and signed
statements from on‐site witnesses and others with direct knowledge of facts
pertinent to the event. Guidance should be given to the persons giving
statements to describe who, what, when, and where in regard to the CMS

Accident. Use of sketches and photographs for positions of people and


equipment is encouraged. Investigation teams may request additional written
statements as the investigation progresses.

The above must also be followed for preservation of information in cases of


high potential Light Accidents and Near Accidents.
5.18 Investigation Reports
The investigation team must prepare a final report and upload it into QUEST.
After the investigation is completed, the investigation leader should debrief
management before leaving the site.

Final investigation reports for all CMS Accidents and other HSE Events defined
in 5.15 shall be completed within three weeks of occurrence.
5.19 HSE Events Review and Closure
Once the investigation is completed, HSE Events shall be reviewed by
management as per the Responsibility Matrix in Appendix A: HSE Event
Reporting and Management Responsibility Matrix. Generally, the senior line
manager is the Review Leader.

Management reviews shall be conducted as soon as possible after HSE Event


investigations are completed. However, the period between conclusion of the
investigation and management review shall not exceed eight weeks.
5.20 Information Sharing and Alerts
After review of a CMS HSE Event is concluded, the corresponding Country HSE
Manager shall ensure that a one‐page slide of the main learning points from the
HSE Incident investigation is prepared. The slide shall present a brief description
of the HSE Incident, the identified root causes, any lessons learned, any
recommended actions to be implemented, and an illustrative graphic related to
the HSE Incident.

The Country HSE Manager is responsible to transmit this slide to the VP‐QHSE
for possible global distribution.
5.21 Mine the Diamond-Focus on HiPo Events
‘Mine the Diamond’ is a term Saxon has borrowed from ExxonMobil’s Safety
Leadership Forum. It is based on the principle that not all Light Severity injuries
should be treated the same, but rather a focus should be placed on HiPo’s. In
Saxon, Mine the Diamond has been expanded slightly and describes a two-step
process to aid in injury prevention:
1. Treat all Light Accidents with a High Potential Risk (Red) in the Saxon
Operations Risk Matrix as if they were an actual CMS Accident (i.e.,

conduct a Root Cause analysis, create an RWP that addresses the Root
Causes, and close out the Action Items).
2. Do the same for Near Accidents and Hazardous Situations with a High
Potential Risk (Red) in the Saxon Operations Risk Matrix

Mine the Diamond is shown in the graphic below. Research over many years
from ExxonMobil shows that only 20% of all injuries has a high potential
consequence. This 20% is captured ‘visually’ in the vertical diamond inside the
traditional pyramid of accident severities. Mine the Diamond means focus on this
20% inside the diamond: eliminate the Root Causes, minimize HiPo’s, and lower
injury rates.

Mine the Diamond shall be achieved in Saxon through the implementation of the
requirements in Section 5.16 above.

5.22 Compliance Monitoring


The Region QHSE Manager is responsible to ensure a compliance audit of this
standard is performed on an annual basis.

Country QHSE Managers are responsible to review all CMSL HSE Events entered
into QUEST for their country to ensure compliance with this standard.

Appendix G: Work Relatedness


An injury or illness occurring in the work environment that falls under one of the following exceptions is
not work-related (i.e., not Industry Recognized) for Saxon reporting and ‘KPI consolidation purposes’,
even if it is considered work-related according to local government regulations.

OSHA
You are not required to record injuries or illnesses as work-related if . . .
1904.5(b)(2)
(i) At the time of the injury or illness, the employee was present in the work environment
as a member of the general public rather than as an employee.
(ii) The injury or illness involves signs or symptoms that surface at work but result solely
from a non work-related event or exposure that occurs outside the work environment.
(iii) The injury or illness results solely from voluntary participation in a wellness program
or in a medical, fitness, or recreational activity such as blood donation, physical
examination, flu shot, exercise class, racquetball, or baseball.
(iv) The injury or illness is solely the result of an employee eating, drinking, or preparing
food or drink for personal consumption (whether bought on the employer's premises
or brought in). For example, if the employee is injured by choking on a sandwich while
in the employer's establishment, the case would not be considered work-related.

NOTE: If the employee is made ill by ingesting food contaminated by workplace


contaminants (such as lead), or gets food poisoning from food supplied by the
employer, the case would be considered work-related.
(v) The injury or illness is solely the result of an employee doing personal tasks (unrelated
to their employment) at the establishment outside of the employee's assigned
working hours.
(vi) The injury or illness is solely the result of personal grooming, self-medication for a
non-work-related condition, or is intentionally self-inflicted.
(vii) The injury or illness is caused by a motor vehicle Accident and occurs on a Saxon
parking lot or Saxon access road while the employee is commuting to or from work.
(viii) The illness is the common cold or flu (NOTE: contagious diseases such as tuberculosis,
brucellosis, or hepatitis A, are considered work-related if the employee is infected at
work).
(ix) The illness is a mental illness. Mental illness will not be considered work-related unless
the employee voluntarily provides the employer with an opinion from a physician or
other licensed health care professional with appropriate training and experience
(psychiatrist, psychologist, psychiatric nurse practitioner, etc.) stating that the
employee has a mental illness that is work-related.
