Dear Sir/Ma’am,

In reference to your request for the Office of Safety Services of the Philippine Red Cross - Laguna City of Calamba
Branch to conduct the following course/s with your office to wit:

STANDARD FIRST AID AND BLS-CPR/AED TRAINING | JULY 18-21, 2022 | 8:00 AM – 5:00 PM

PRC Laguna confirms that it shall:

1. Provide one/two instructor/s at any given time to conduct the above course/s;
2. Provide training equipment for the duration of the training;
3. Issue a Company Certification (provided at least 1 participant passes) containing list of passers of the evaluating
examination – at no cost;
4. Will issue within 5 working days, upon individual request and clearance from sponsoring company, a Certification
to participants who passed the evaluating examination;
5. Evaluate the written and skills examinations of participants and issue Certificates of Completion through a form
of a license card to those who passed the course within 14 days after the training;
6. Provide a Membership ID within 14 working days after the training;
7. Issue an official receipt upon payment of the course/s fee.
8. Provide the disinfection solution for the duration of the training.

LF PHILIPPINES INC. confirms that it shall:

1. Ensure that the training area to be used is thoroughly cleaned and decontaminated prior to the training day;
2. Ensure participants of not less than 14 or not more than 25 participants. Each participant shall have a Participant’s
Assessment Form which will be sent through email to laguna@redcross.org.ph;
3. If the minimum number of participants is not met, the requesting party shall pay the amount for the minimum
of 14 pax;
4. Remit the amount of (Php63,000) before the scheduled training date. If check, payable to: PHILIPPINE RED
CROSS - LAGUNA CHAPTER .NO PAYMENT, NO TRAINING POLICY will be reinforced.
6. Provide LCD projector and desktop/laptop for the lecture/training;
7. Provide meals and snacks (am and pm) to the instructors;
8. Provide daily transportation of instructors from and to Philippine Red Cross - Satellite Office in Calamba City;
9. The agreement letter or conforme shall be signed and paid in full a month prior to the scheduled training
date. Full payment amount shall be based on the forecasted number of participants for the training.
10. Cancellations shall incur fees and non-refundable . 50% cancellation fee within 2 weeks before the training
date, and 100% cancellation fee if within one week before the training date.

Non-compliance with the training requirements by the requesting party gives the right to the Philippine Red Cross –
Laguna Chapter to terminate the training/services. Likewise, PRC is Tax exempted by the BIR.
