OBSERVATIONS:
I. Significant behavior’s (Related during sessions/dialogues/observations/portrayed during her
stay at Haven:

A. Positive (Mga positibong pag-uugali,pananaw mga plano sa buhay, kios, Gawain at iba)
 She cooperates in every activity given by the center especially during group session and
livelihood training.
 She always makes her family happy even her case was not yet finish.

A. Negative (Mga negatibong pag-uugali,pananaw mga plano sa buhay, kios, Gawain at iba)
 She sometimes wakes up late in the morning because she felt that her buttocks were in pain
and her cesarean portion of her stomach.

II. Relationship with peers (Paano siya makitungo o makipagkapwa tao sa kasamahan)
 She gets along well with her co clients especially in the activity given by the center.

III. Relationship with older people/staff (Paano siya makitungo sa nakakatanda/sa staff)
 She shows respect to the staff by asking sorry if she did something wrong like she woke
up late in the morning.

IV. Health status of the client (Lagay ng kalusugan at pangangatwiran)

 She’s always complaining about her buttocks and her cesarean portion of her stomach.

V. Recommendations/Suggestions
 She should be given counselling by the psychologist because of her different attitude.
