Introduction

A questionnaire is a researching tool used in surveys, there are multiple types of


questionnaire which will be explored in the present work, each one is adequate for a specific
purpose or context of research. This work is informative and brings concise information
about questionnaires, taken from reliable sources. The focus of the work is in exploring all
the information available related with questionnaire in a way that is clear and easily
understandable.

The present work not only explores the topic conceptually, but also presents practical
examples of the use of questionnaires, presenting multiple types of questionnaire and it’s
sample, in order to make the information clearer.

1. Questionnaire
A questionnaire is a list of questions or items used to gather data from respondents about their
attitudes, experiences, or opinions. Questionnaires can be used to collect quantitative and/or

Are commonly used in market research as well as in the social and health sciences. For
example, a company may ask for feedback about a recent customer service experience, or
psychology researchers may investigate health risk perceptions using questionnaires.

1.1.Rationale

During the researcher observations in Universidade Rovuma in the morning period, the
researcher noticed that very quite students are out of their line using platforms or network in
classes. In this perspective, the researcher decided to investigate the possible reasons behind
the low active of unknown access of the network in the classroom. The findings from this
research could be useful to: teachers, directors, headmaster of the university, technologics in
order to improve students technology acknowledgement ; so that students can be able to
master a lot of thing about communication and information system to a high level and
professional.

1.2. Questionnaire design

A questionnaire should take a careful consideration therefore needs to be given to the design
of the questionnaire. All questionnaires should take have the following:

The title

All questionnaires require a title. It needs to be short, simple, appealing and inviting, not the
full academic title of the study.

The identifier

Each questionnaire will also probably require a participant identifier. If your survey is
confidential i.e. you know the identity of each respondent but their identity is confidential to
you, then all questionnaires will require a confidential unique identifier. Names and
addresses should not appear on the questionnaire itself. If however your survey is
anonymous then you cannot know the identity of any of your respondents and none of your
questionnaires should have an identifier.

Instructions

It is crucial to include instructions on the questionnaire, in terms of ticking boxes, circling


numbers, allocating priority order to a list, etc. If you are doing a postal survey or a
questionnaire for selfcompletion, you will need clear instructions for the respondent.
Alternatively, if using interviewers you will need to provide them with instructions in terms
of filtering and what to read out, etc.

A survey is a research method where you collect and analyze data from a group of people
whereas a questionnaire is a specific tool or instrument for collecting the data.

2. Types or methods of questionnaire


2.1. Based upon of the responds required

1. Closed- ended or multiple choice or fixed ended questionnaire are questions offer
respondents a fixed set of choices to select from. And this are best for collecting data on
categorical or quantitative variables.

1.1. Categorical variables of closed ended this can be nominal and ordinal variables

1. Nominal variables include categories that can’t be ranked, such as race or ethnicity. This
includes binary or dichotomous categories.

2. Open- ended or restrict choice or long form

Are questions that allow respondents to give answers in their own words. Because there are
no restrictions on their choices, respondents can answer in ways that researchers may not
have otherwise considered.

2.2. Based upon the method of administer

1. Mail or self-administered can be delivered online or in paper-and-pen formats, in person


or through mail. All questions are standardized so that all respondents receive the same
questions with identical wording.

2. Face to face or researcher or interviewer administered are conducted by an
interviewer asking questions of a respondent in person which after having answered you
need to give to comment or give your own argument. And his interviews can take place
by phone, in-person, on paper or on online between researchers and respondents.

Conclusion

Questionnaire is a set of questions or items used to gather information from respondents, it’s
a concise work with the focus on bringing information about questionnaire. However the
present work doesn’t cover all the information about the topic for the lack of sources,
therefore more research is needed. Trough the work is possible to understand the
conceptualization of a questionnaire very easily, in which, we could explore multiple types of
questionnaire and observe it’s practical use therefore reinforcing the explanations.

It is clear at this point that a questionnaire is a very useful tool for the researcher to find out
information directly from the subjects, and therefore a more facilitated method for the
research to be done. Various types of questions were presented in this work, so it will be up
to the researcher to define what type is more adequate for his researching context.
