Failure to include the bar code sheet may cause a delay in the processing of your case. The bar code sheet should be placed on top of all documents being submitted for this case and mailed to the address provided. ______BIRTH CERTIFICATE:___________________________________________ Please submit this applicant's original or a certified copy of the original document listed above. Each applicant will need to obtain an original birth certificate issued by the official custodian of birth records in the country of birth, showing the date and place of birth and the parentage of the alien, based upon the original registration of birth. The certificate must contain the: Persons date of birth Persons place of birth Names of both parents, and Annotation by the appropriate authority indicating that it is an extract from the official records If the certificate is a certified copy of the original, it must come from the issuing government authority and it must contain annotation by the issuing government authority indicating that it is an extract from the official records. Documents must be from the appropriate issuing government authority. Failure to provide the correct document will delay processing of your case and may result in re-scheduling of your interview, or refusal of a visa. In addition to sending the original documents or certified copies of the documents to the NVC, a photocopy of the original documents must be submitted. Unobtainable birth certificates If the applicants birth record is unobtainable, please obtain a certified statement from the appropriate government authority stating the reason the applicants birth record is not available. With the certified statement the applicant must submit secondary evidence. For example: A baptismal certificate that contains the date and place of birth, as well as both parents names (providing the baptism took place shortly after birth) An adoption decree for an adopted child An affidavit from a close relative, preferably the applicants mother, stating the date and place of birth, both parents names, and the mothers maiden name. Suitability of documentation for immigration purposes

For many countries, the Department of State has provided country-specific criteria for ensuring that applicants submit visa processing appropriate documents. Please follow the instructions below to determine if the Department of State has issued any country-specific guidelines for the requested document. Please go to www.immigrantvisas.state.gov. 1. Click on the Fees & Reciprocity Tables box located on the left side of the page. 2. Click on the Visa Issuance Fee - Reciprocity Tables line near the middle of the page. 3. From the drop down menu, select the country where the document was issued. 4. Scroll down to the document type to view the specific criteria. All documents not in English or the official language of the country in which the visa will be applied for must be accompanied by a certified translation. Your translation must include a statement signed by the translator that states the following: * Translation is accurate, and * Translator is competent to translate. ______PASSPORT:____________________________________________________ Please submit a copy of the biographic data page of this person's passport, including the expiration date of the passport. The passport must be valid. ______PHILIPPINES POLICE CERTIFICATE:______________________________ POLICE RECORDS Available and reasonably reliable. Philippine-born immigrant visa applicants 16 years and over are required to furnish certificates issued by the National Bureau of Investigation (NBI). The green NBI certificate annotated for travel abroad is the correct certificate. Other colors are for local use only. Foreign-born applicants who have resided in the Philippines for six months or more may be issued NBI clearances upon application either in person or by mail. Applicants residing outside the Philippines should send requests for certificates together with money orders or bank drafts for the fees and postage to IRD-Mailed Clearance Section, National Bureau of Investigation, Taft Avenue, Metro Manila, Philippines. Philippine Embassies abroad can assist Filipinos in obtaining NBI police clearances. The NBI has its own fingerprint charts and personal data forms that it will send to the requesting applicant for execution. Most applicants abroad find it more expeditious to have a relative or friend in the Philippines assist them in obtaining their certificates from the NBI. In that instance, a letter of authorization is required. The applicant may also submit a copy (personal copy) of a prior NBI clearance to facilitate the processing of a new clearance. Posts are advised that not all arrests and convictions occurring outside of Manila are reported to the NBI. NBIs criminal records database is not foolproof and the remarks provided on reports require explanation.* Manila and major Philippine cities have computerized records.
