CONTRACT OF SERVICE
KNOWN ALL MEN BY THESE PRESENTS:

This contract of service is entered into by and between DEPARTMENT OF EDUCATION -

DIVISION OF LANAO DEL NORTE with office address at Gov. Arsenio Quibranza Provincial
Government Compound, Pigcarangan, Tubod, Lanao del Norte, represented by its SCHOOLS
DIVISION SUPERINTENDENT, EDILBERTO L. OPLENARIA, CESO V, herein referred to as the
“FIRST PARTY”;
-and-
ALIAH S. SARANGANI INUDARAN II, SAPAD
_________________________, a resident of _________________________, Lanao del Norte,
contracted as Ustadz/Teacher for the Madrasah Education Program or ALIVE (Arabic Language
and Islamic Values Education INUDARAN
Program ESat SAPAD
______________________________,
_______________________ District, herein referred to as the “SECOND PARTY”; do hereby agree to
the following conditions:

A. Responsibilities of the First Party:

1. Supervises and evaluates the Asatidz/ALIVE teachers to ensure that classes are
implemented according to the Madrasah Curriculum.
2. Conducts regular monitoring and technical assistance to ensure that planned
activities indicated in the school’s Annual Improvement Plan are implemented
accordingly.
3. Provides necessary support to the Asatidz/ALIVE Teachers.
4. Certifies attendance and payment for the honorarium/allowance of Asatidz/ALIVE
teachers.
5. Maintains active linkages with the LGU and other interested GO’s and NGO’s to
draw-in support to the Madrasah Education Program.

B. Responsibilities of the Second Party:

1. Teaches/handles ALIVE classes under the Madrasah Education Program with at
least 15 children per class using appropriate and innovative teaching strategies
2. Renders compulsory service in the school of assignment for at-least four (4) hours a
day from Monday to Friday, handling a minimum of 3 classes on Arabic Language
(3x a week) and Islamic Values Education (2x a week) with a period of 40 minutes
per class for a total of 120 minutes in a day
3. Facilitates learning in the elementary/secondary schools through functional lesson
plans (for new teachers up to 3 years) Daily Log (for teachers teaching 4 years and
above) of activities and appropriate, adequate and updated instructional materials
4. Monitors and evaluates pupils/students’ progress in accordance with DO No. 8, s.
2015
5. Undertakes activities to improve performance indicators
6. Maintains updated pupils/students’ progress regularly
7. Supervises curricular and co-curricular projects and activities
8. Maintains updated pupil/student school records and such other report needed
 by the school/district
9. Counsels and guides pupils/students
10. Supports activities in the school and district, including governmental and
non-governmental organizations
11.Conducts Action Plan and participate in summer activities
12.Maintains Daily Routine (classroom cleanliness, classroom management, overall
physical classroom atmosphere)
13.Maintains harmonious relationship with fellow teachers and other school personnel
as well as with parents and other stakeholders
14. Submit to the school and district office needed reports which include, but not
limited to Muslim and ALIVE enrolment, updated Asatidz and School ALIVE profile
and Year End ALIVE clearances among others.
15.Does related work

C. Miscellaneous Provision:
1. Payment of Seven thousand (Php 7,000.00) honorarium and One thousand (Php
1,000.00) IMs allowance per month for Twelve (12) months from January to
December to the qualified Asatidz, taken from the Madrasah Education Program
(MEP) Fund in accordance with DO No. 41, s. 2017 and DO No. 25, s. 2021.
2. That this contract maybe recalled before expiration of term on the grounds that if
funds are no longer available, or when the services are no longer needed and such
other valid grounds to nullify this contract.
3. Renewal of this contract shall be based on the performance during the School Year
20___
21 - 20______.
