1- HSE Action Due date

1.1 Safety Moment

Entry to any Aramco Facilities shall not be allowed without full immune against Covid-19.

All remaining workers shall be Immune before the next meeting (20-Feb-2022)

All Workers must be vaccinated with 3 doses of vaccine. (27-Mar-2022) MASAFI 27/03/22

The presence of the HSE Supervisor from MASAFI in the Weekly Meeting is mandatory.

Civil Supervisor CV will be submitted this Week before 24-Mar-22

1.3 Method Statement and JSA

A MASAFI is requested to provide Initial Method Statement and JSA.

JSA shall be submitted along with each MOS as a unique document.

JSA-8 Nos were received by Masafi and 7 Nos were approved by Aramco.

MOS-10 Nos were received by Masafi and 8 Nos were approved by Aramco

Excel log for MOS and JSA was sent by a MASAFI.

1.4 Availability of HSES

1.5 Material Transportation to BS-2

SICIM informed MASAFI to send prior notification (HSE + Const.) for any material
transferring to BS-2.

No materials are to be transported from Agdah Road, the only approved traffic route for
material transportation to BS-2 is from Marabah Dam (as shown in SICIM Traffic and
Journey Management Plan).

Always complete the “JAPL-F-HE-024 Load Transportation Checklist” before starting any MASAFI INFO
material movement to BS-2. SICIM HSE Representative signature is compulsory for
validation of this checklist.

It was reminded to MASAFI to mobilize Boom Truck for Transportation of Material at BS-1 &
BS-2.

SICIM inform MASAFI to arrange the Ambulance along with Male Nurse at BS1 & BS2. (20
Mar-22)

1.6 Document for Heavy Equipment and Operator’s Certificate

Stated that all Heavy Equipment & operators are certified as per SA GI 7.025, following
documents shall be made available along with HE / Operators

- Valid Equipment Insurance

- Valid 3rd party inspection sticker

- Valid Iqama / Residential permit

All drivers and heavy equipment operators in mountainous areas shall be evaluated by
SICIM HSEM & CM before mobilization to site, authorization pass shall be given to
driver/operator after qualifying practical test. In addition, Vehicles and equipment are to be
ensured for suitability in the mountainous areas.

1.7 Training

MASAFI to ensure all employees arriving at project sites shall be scheduled with HSE
training (Mandatory & Job-specific). Employees shall not be mobilized to perform any
activity without the mandatory HSE induction Course offered by SICIM. MASAFI CO Shall
conduct daily TBTs to discuss risks associated with the job and potential hazards.

Communication with Sicim only one focal person by email regarding training and schedules

Covid-19 Vaccine two-dose is a must for a safety orientation.

Driver for BS2 shall go for assessment and test for driving in the mountain area.

Power Tools Inspection and Training for workers to be arranged as a priority.

SICIM informs MASAFI to take action to control traffic violations at BS2 and follow SICIM
Traffic Journey Plan.

1.8 Welfare facility and safety concerns

The MASAFI confirmed Welfare facilities will be provided within one week. (8/12/2021).

MASAFI confirmed that the Waste Management agreement shall be finalized on or before
29-12-2021.

MASAFI informed that Waste Management Facilities and office will be mobilized at BS-2
before the next weekly meeting. (30-01-2022) next Date 06-03-2022

MASAFI informed that Waste Management Contract not yet closed and Facilities & Office
for BS-2 will be mobilized before next weekly meeting (28-02-2022) ongoing negotiation
with RISAL

1.9 Medical Coverage/Insurance

Certified First aider shall be provided by the MASAFI as per Saudi MOH requirement a ratio
of 10:1 shall be maintained. A Minimum of 36-Unit First Aid Cabinet shall be readily
available near the worksite.

Masafi informed that 3 people will get Certification before the next meeting (20-Feb-2022)
MASAFI OPEN
Agreement with the health facility/ hospital is already done and the MASAFI will provide a
copy of the agreement by 6/12/21.

The MASAFI has sent valid insurances, which are going to expire end of December 2021.
MASAFI informed that insurance auto-renewed before expiring one week. (Closed)

Masafi will send a new Medical Insurance policy to Safety Manager 20-Mar-22.

1.10 Camp and TCF Inspection

Internal Inspection of Camp and TCF Will be on Monday 22/11/21 at 9 o clock.

Closeout report of observations shall be submitted by 05/12/21 and Aramco Inspection by


09/12/21. MASAFI 21/11/21

Aramco Accepted the Camp Facility of Masafi and advised to maintain the same
requirements at all times.

MASAFI to prepare New Building for Inspection by SICIM HSE.

The contractor was informed to provide MTC load test reports and manuals before starting
of scaffolding activity.

SICIM Inform MASAFI for submitting the Scaffolding Plan before 3 Weeks to start
scaffolding activities.

1.12 Permit to Work

Current Situation regarding Permit to Work System was created by Masafi So Masafi shall
arrange Work Permit Receiver and Permit from ARAMCO before starting any activity.

1.13 Closure of Observations

Masafi to Close all outstanding Observations of ARAMCO before 17-Feb-2022

2.1 CV of Qc Inspectors (SICIM QCS to update)

The MASAFI is requested to provide the CV of the Qc Inspector per each discipline by
work schedule and needs 30 days in advance.

Following Qc Inspectors are foreseen in the project:

Civil QC: Sabir Ali refused to join Masafi that’s why his Aramco Interview was cancelled.
2nd QC Civil Syed Hammad Ullah CV submitted and waiting for an Interview.
3rd QC Civil Raja Saqlain CV received but his agreement not finalized with MASAFI once
will finish then SICIM Proceed for interview.

Plumbing & HVAC: The CV of the HVAC/plumbing inspector has been already approved
and mobilized by SICIM on behalf of Masafi.

Coating Crew Supervisor and Applicator:


Coating Crew Supervisor and Applicator shall be qualified according to SAEP-316 for the
coating system listed in SAES-H-101V provided by Masafi.
MASAFI to coordinate and finalize with Sicim’s recommended Supervisor and Applicator.

RTR Laminator: Masafi shall organize Certification Program RTR Laminators shall be
provided by Future Pipe (RTR Manufacturer). The certification Process shall be started
immediately and Laminators shall be mobilized at the site before 25-02-2022.
Jawad will be sending the documents of the RTR laminator and supervisor to the Sicim Qc
manager

E&I: CV will be submitted by mid of January 22. (OPEN) recommended CV provided by
SICIM. CV to be submitted by 27-Mar-2022.

Masafi shall provide enough QC inspectors to cover all the inspections at BS1 and
BS2

Each inspector must have a CBT card and previous experience with Saudi Aramco.

2.2 Calibration certificates

The calibration certificate of each tool/device/equipment shall be sent to the Contractor


QA/QC Department.

MASAFI to submit a list of tools, devices, and equipment to be calibrated. INFO 15/11/2021

MASAFI shall make sure of the on-time availability of RICI technicians for Testing required
at the site.

The contractor asked the MASAFI to provide an Excel log of testing equipment to monitor
expiry dates. (OPEN)

MASAFI shall have a Calibration Procedure and required to submit to SICIM for info

MASAFI shall attach Calibration log (share the controlled document through mail on
every Saturday with SICIM) updated revision – along with MOM.

2.3 Batching Plant and Concrete Mix design (CIVIL QCS to update)

The MASAFI is requested to provide all required documents to get Aramco’s approval for
the batching plant.

2nd Volumetric Batch Plant and Concrete Pump are mobilized at the site on 09-02-2022. MASAFI/

RAW MATERIAL for Concrete Shall be stored near to Building to avoid delay during
pouring.

2.4 Inter-discipline Clearance for all Concrete Pouring

Inter-Discipline Release/Clearance is required before proceed any Concrete Pouring for

Accommodation Buildings.

Masafi to submit Inter-discipline Clearance form to Sicim for review and concurrence.

2.5 Quality System Deliverables

SICIM QAM informed MASAFI that the service provider is having a contract exceeding 6
months duration.

Hence, MASAFI will be monitored for quality deliverables compliant to Sch. Q, as applicable MASAFI
and will be subjected to internal quality audits as applicable and required by Sch.Q OPEN
requirements.

MASAF (Project Manager & Quality Lead) agreed and noted the deliverables to be
submitted.

1. MASAFI should submit a Site Quality Control Procedure for review and approval of
SICIM QA/QC.

The QCP shall address the brief scope of work, construction schedule, quality
planning, responsibilities & authorities clearly defined, quality documentation related
to work procedures / MOS, materials, preservation, resource planning, testing, and
inspection requirements. Informed and explained to the QC Lead related to the
required elements to be addressed and incorporated

2. As a minimum, the following documented information shall be attached to the QCP

3. Daily RFI schedule shall be e-mailed to MASAFI Project manager and keeping
SICIM (PM/CM/QCM/QCS) in the loop

4. Two Weeks Look Ahead Inspection schedule (aligned to construction schedule)


shall be electronically submitted (as per SICIM Format) – every Thursday (covering
activities in the following week)

5. Controlled/updated Calibration xl sheet shall be submitted weekly as an attachment


to MOM

6. Monthly Quality Dashboard / Report shall be submitted on or before 05 th of every


start of the month

7. MASAFI shall strictly comply with Aramco approved Project Quality Plan as
applicable to their scope of work.

MASAFI shall strictly comply with Aramco approved (SICIM) NCR Procedure

2.6 Pro-active Approach to face Upcoming 60% Construction Quality Audit: OPEN

 SICIM informed that MASAFI should conduct internal assessment (with the support of
SICIM QCS) and identify the quality findings related to ongoing construction works on 15-Jun
their SOW, on or before 15-Jun-22

 SICIM requested MASAFI to determine the root cause of all deficiencies found during
internal assessment and apply proper corrective actions to resolve on or before 25-Jun- 25-Jun

 SICIM informed that Aramco approved 3rd party Lead Auditor will conduct quality audit
on construction scope of work (on SICIM & service providers) around 25-Jun-22 and Action
hence the above actions need to be completed to minimize external Needed

3. Procurement Action Due date

3.1 MASAFI shall send BOQ to Contractor PQS for material verification approval with
relevant 9 COM. (OPEN)

Material List not yet submitted MASAFI promised for this week (02/12/21)

The MASAFI is reminded about the different steps of inspection regarding Vendor and
Project inspections.

Approval of Project Materials (Bill of Material) shall be the BOQ with applicable 9COM
number to verify the vendor inspection requirements, any RVL controlled materials, if you
have in procurement, shall be prepared dedicated material requisition (MR), with Sicim MASAFI 17/11/2021
Format. For contractor and company’s VID review and approval. Before placing the order,
you shall get NO Objection from VID to place the order.

All project material shall follow the procurement process for each level of inspection as per
SAEP-1150 and 1051.

MASAFI to provide Procurement plan and Schedule level III by 28/11/21. (OPEN)

The Contractor points out that the MASAFI didn’t send any updates on the Procurement to
date.

3.2 MR Submission and approval Status

Updated Status shall be provided in the weekly meeting.

MASAFI to follow up for MR status to Procurement of SICIM in Khobar office.

Concrete Repair to be submitted with attached Approved Vendor, Material Name & Material MASAFI/

3.3 Material Delivery at site

3.4 Transmittal

The attachment Issue shall be discussed and solved with Mr. Riyaz IT Engineer of Sicim.

system protection policy, so Masafi is asked to use another platform.

4. Contstruction / Engineering Action Due date

Precast drawings / Shop Drawings

Shop Drawing of CMU for BS-1 is submitted by PMT.

Shop Drawing of CMU for BS-2 to be submitted by MASAFI on 02-June-2022.

MASAFI shall submit Shop Drawings of Penetration/Openings in Slabs or Walls for MEP in
3weeks up to 13-03-2022.

4.1 Rebar Fabrication

MASAFI shall compare BBS and Rebar arrangement details for Foundations and Concrete MASAFI Info
Structure, if any discrepancies Red Mark drawing to be submitted and approved before
inspection and pouring.

4.2 Manpower for BS-1 & BS-2

The contractor mentioned mobilizing separate Manpower to cover upcoming activities for
BS-2 and also the Contractor informed Masafi to increase the manpower to start work MASAFI 30-01-2022
parallel at both locations BS1 and BS2.

MASAFI to comply with Manpower as per Histogram by end of this week. 17-Feb-2022

4.3 Material Resulting from the Excavation

The Contractor informs the MASAFI that the material resulting from the excavations in
progress must be transported to the appropriate landfills as indicated in Attachment I item
3.1 & 3.2.

MASAFI will be responsible for the disposal of extra soil from the project location.

4.4 Disposal of Excavated Soil

The MASAFI will use the excavated material for Backfilling (after Screening) and Extra
Material Shall be disposed of from the Site by MASAFI.
