BIR Change of Business Name

Documentary Requirements:
1. Department of Trade and Industry Certificate of Registration (Updated)
2. BIR FORM 1905 (Application for Registration Information Update/Correction/Cancellation )
3. BIR Certificate of Registration (Original)
4. Surrender Unused Official Receipts
5. Notarized Special Power of Attorney
BIR Registration for Sole Proprietors
Documentary Requirements:
1. BIR Form 1901 (Application for Registration for New Business) (2 original copies)
2. DTI Certificate of Registration (Photocopy)
3. Mayor’s Permit
4. Any Government ID of the Sole Proprietor (Photocopy)
5. BIR Form 1906 (ATP Application) and Prienter’s Job Order
6. Registration Fee and Doc Stamp - ₱530.00
7. Authority to Print Receipts (ATP) – minimum cost of 10 sets duplicate (50pcs/set) is ₱1,800
8. Registration of Books of Accounts – ₱100 for 2 books (journal & ledger)
9. If application via Representative:
a. Notarized SPA
b. Any valid ID of Authorized Representative (Photocopy)
BIR for Closure of Business or Cancellation of TIN/Registration due to Dissolution
Documentary Requirements:
1. BIR Form 1905 (2 original copies)
2. List of Ending Inventory of goods, supplies, including capital goods (1 original)
3. Inventory of Unused Sales Invoice/ Official Receipts (1 original)
4. Unused Sales Invoice/ Official Receipts and all other unutilized accounting forms (e.g
vouchers, purchase orders, debit/ credit memos, delivery receipts, etc.) including business
notices and permits as well as COR shall be subject for destruction to be witnessed by BIR
personnel and officials
5. If application via Representative:
a. Notarized SPA (1 original)
b. Any valid ID of the Taxpayer and Authorized Representative (Photocopy)
