AFFIDAVIT OF SELF ADJUDICATION AS SOLE HEIR

I, ABEGAIL A. CUEVO, Filipino, 25 years old, single, with residence and post
office address at Brgy. Rizal, San Leonardo, Nueva Ecija, after having been duly sworn
to an oath in accordance with law do hereby depose and state:

1. That I am the only surviving heir/child of AVE MARIA ANABE CUEVO who died
last November 12, 2010 in San Leonardo, Nueva Ecija, as evidenced by her
Death Certificates, copy of which is hereto attached as Annex “A” and made an
integral part of this affidavit;

2. That at the time of the death of Ave Maria Anabe Cuevo she left no debts but left
a parcel of land located at Poblacion, San Leonardo, Nueva Ecija with an area of
Four Thousand Seven Hundred Six (4,706 sq.m.) Square Meters, embraced in
and covered by TCT No. 041-NT-243122 of the Registry of Deeds for the
Province of Nueva Ecija, more particularly described as follows:

“A parcel of land (Lot 1465-B of the subd. Plan, Psd-03-005792,

being a portion of Lot 1465, San Leonardo Casd., LRC Rec. No. ),
situated in the Poblacion, Mun. of San Leonardo, Prov. Of Nueva Ecija.
Bounded on the NE., along line 1-2 by Lot 1466, San Leonardo Cad., on
the SE., along line 2-3 by Irrigation Canal; on the SW., along line 3-4 by
Lot 1464; and on the NW., along line 4-5 by Lot 1474, both of San
Leonardo Cad., and on the NE., along lines 5-6-1 by Lot 1465-A of the
subd. Plan. x x x containing an area of Four Thousand Seven Hundred
Six (4,706) Square meters. x x x “

3. That as the only surviving compulsory heir, I am entitled to inherit the above-
described parcel of land;

4. That undersigned is willing to pay such taxes as maybe imposed by the Bureau
of Internal Revenue and other government agencies, as provided by law;

5. That pursuant to Rule 74, Section 1 of the Rules of Court, I hereby adjudicate
unto myself all the rights, interest and ownership over the above-described parcel
of land by means of this affidavit and hereby file the same with the Office of the
Register of Deeds for the Province of Nueva Ecija in Cabanatuan City with the
request that said adjudication be made effective without judicial proceedings as
prescribed by the aforementioned Rules of Court.
