JOINT AFFIDAVIT
I, JENNIFER C. PEREZ, of legal age, Filipino, widow and a resident of Talibaew,
Calasiao, Pangasinan, after having been duly sworn to in accordance with law hereby
depose and state:

1. That I am legally married to MAXIMO SONIEGA PEREZ on October 18, 1999,

copy our marriage certificate is hereto attached as Annex “A”;
2. That prior to my marriage with my husband MAXIMO SONIEGA PEREZ, he was
previously married with CORAZON CAUINIAN PEREZ who died on January 1,
1999, a xerox copy of her death certificate is hereto attached as Annex “B”;
3. That my husband is a pensioner of the Social Security System;
4. That on April 20, 2022 my husband died due to pneumonia, copy of his death
certificate is hereto attached as Annex “C”;
5. That upon the death of my husband, I immediately reported the fact of incidence
to Social Security System to claim his burial claims;
6. That it was discovered that the death of CORAZON CAUINIAN PEREZ has
never been reported to the said national agency and that my husband, as the
legal beneficiary of the former, continued to receive pension from the SSS up to
the time of his death;
7. That as the legal heir of MAXIMO SONIEGA PEREZ, having left with me three
(3) children, two (2) of which are still minors, I filed with the SSS a claim for
monthly pension;
8. That I was advised by the said national agency to secure an original copy of the
death certificate of CORAZON CAUINIAN PEREZ from the Philippine Statistics
Authority because as per record of the SSS, CORAZON CAUINIAN PEREZ is
still the declared beneficiary of my husband;
9. That when I went to the PSA to secure an original copy of the death certificate of
CORAZON CAUINIAN PEREZ, my request has been denied for the reason that
there is a need to present an authorization from the nearest of kin of the
deceased;
10. That despite my repeated request to PERPETUA PEREZ FRONDA, the only
daughter of CORAZON CAUINIAN PEREZ and my husband, to authorize me to
secure a copy of the death certificate of her mother, she vehemently refused to
do so, for unknown reason;
11. That I am executing this affidavit to attest to the truthfulness of the foregoing
statements and in order to secure an original copy of the death certificate of
CORAZON CAUINIAN PEREZ from the PSA.
