To whom it may Concern:

THIS IS TO CERTIFY THAT based on the data and information available in

the Municipal Disaster Risk Reduction and Management Office, the fishing pump
boat including the fishing paraphernalia of MR. LUIS A. MANGAYA-AY, of legal
age, and a resident of Purok 4, Gamuton, Lanuza, Surigao del Sur, has been totally
damaged due to the strong winds and floods caused by Typhoon Odette on
December 16, 2021 at Barangay Gamuton, Municipality of Lanuza, Surigao del
Sur.
This is to certify further that such pump boat and fishing paraphernalia are
significant equipment and tools in the livelihood of the above-named individual.

This certification has been issued upon the request of Mr. Mangaya-ay for
any legal and appropriate purposes it may serve.
