PARENTAL CONSENT

I/We hereby willingly and voluntarily give consent the

participation of my son/daughter

School-Based Press Conference at Sta. Ana Central School.

I have considered the benefits that my son/daughter will

acquire from his/her participation in this activity provided that
due care and precaution will be observed to ensure the safety of
my son/daughter and that Sta. Ana CS employees and personnel may
not be held liable and responsible for any untoward incident that
may occur beyond their control.
