Example:
You want to know what ornamental plants can be planted
that is suitable in our environment.
When you were in Grade 4, one of your lessons in Agriculture was

Our Lesson for today is about on how to create and process an


Online Survey.

Let us know first the definition of an Online Survey.

An online survey is an innovative, easier, and faster way to deliver


surveys, collect results and analyze results using an online system.
This tool is used to define questions and possible answers using the
internet. The link to these survey questions is then sent to the
participants who will answer the survey online.

Steps on how to Create a Google Form/Google Survey


Form.

1.To create an online survey using Google Forms, you must have the
following:
● Cellphone or laptop with internet access
● Google account
● Chrome web browser

2. Log in to your Google account. Click on the FORMS web app icon.
3. Select Blank. Click untitled Form. Type the title description.

4.Accomplish survey setup.


Write a title for your survey. Explain it in two sentences.
There is three checkbox options display at the top of the survey form:
● Sign in to view the form.
● Users are allowed to add responses.
● The respondent’s username is automatically collected.

5. Add and edit questions.

Steps on how to Process a Google Form/Google Survey


Form.
1.Go to Google Form. Click the form you created. Then, click
responses.

2.Click the Google Sheet icon to see the spreadsheet

3. Download the spreadsheet and see the result.
