SUBJECT : After Operation Report Re:Shooting Incident resulted to the Death of Pablo
Sagmayao

DATE : June 24,2022

1. Authority : PNP BALBALAN

2. Mission : To arrest Juan Sannadan the suspect of the shooting Incident resulted to
the Death of Pablo Sagmayao and case of Homicide in violation of Article 249 of the Revised
penal Code.
3. Period Covered : At about 5:10 PM of June 24, 2022

4. Area of Operation : at his house located at sitio lower, Brgy. Balbalan proper, Balbalan,
Kalinga.

6. Chronological events :
a. Background:
In June 24, 2022 at about 3:00 p.m., a shooting incident transpired at brgy. Balbalan proper,
balbalan, kalinga , wherein victim one Pablo Sagmayao , 60 years old , married, farmer, a
resident of sitio Guisang, Balbalan proper, Balbalan, kalinga was shot by suspect Juan
Sannadan , 50 years old, married , farmer and a resident of sitio lower, balbalan proper,
balbalan, kalinga.
b. Intelligence Build up:
Balbalan MPS conducted investigation in order to verify and validate the report on the
alleged shooting incident . Result of Investigation revealed that subject was involved in the
shooting incident .
c. Narrative account of operational activities:
On June 24, 2022 at about 4:30 PM, Personnel from PNP BALBALAN, led by PCINSP.GIAN
KRISTOF N. PASUDAG ,arrived in BALBALAN MPS and coordinate to arrest the subject.
 At about 5:00 PM on the same Date , the SW was jointly implemented by the personnel of
PNP BALBALAN as entry Team that was tasked to occupy the area ,takes control and secured
it , Perimeter/security team that was tasked to secure the outer perimeter area and
maintains ground control and PCINSP GIAN KRISTOF N. PASUDAG tasked to perform the
search.

During the implementation , the knocked and announce rule was been observed by the
operating troops , however subject occupant is refused admittance to the said search , thus
they forcibly entered into the said house pursuant to PNP Police Operational Procedure
Manual rule 14 para states that the operatives may break open any outer or inner door or
window or any part of the house therein to implement the warrant.

Thereafter, the search and Investigation Team of BALBALAN MPS composed of PCOL MARK
JELLICO C. NGAYA-AN, PCINSP GIAN KRISTOF N. PASUDAG,SPO1 ROBERT P. BAUTISTA, SPO2
CARDO A.DALISAY,PO2 MARIO D.SUPER, PO2 ATHAN D. REYES,PO1 ROMEL T.ORDIO entered
to the house therein to implement the warrant.

The implementation of SW was attended by the witness, Von N. Gaano, Medical personnel,
Anne delos Reyes, DOJ Representative , Brgy Captain Leonardo P. Montero and Brgy
Kagawad Orlando Cabantug.

Thereafter, Juan Sannadan together with the confiscated pieces of evidence were brought
to BALBALAN MPS for proper disposition.

7. Comments:
Based on the foregoing, it is proper that this case will be in Full-blown trial at the
proper court to determine whether or not respondent could be held criminally of the crime
charged.

8. Assessment:
Since the Respondent was many witness of his crime, it is presume therefore that he
has involvement in the shooting Incident ,Unless otherwise it will be dismissed from the court
for lack of probable cause.
