SUBJECT : Shooting Incident

DATE : December 17, 2014

1. On December 16, 2014 at about 10:00 in the evening at Kurikong KTV Bar located in
Bagong Ilog, Sta Cruz, Laguna shooting incident took place wherein the victim was
identified as one Mario Egay Dugay, 29 years old, single, Criminology student of Don
Pilapil College, Manila and a resident of Bgy Sibol, Pila, Laguna.

2. Initial investigation revealed that while the victim together with his friends were
having a drinking spree at Kurikong KTV Bar located at Bagong Ilog, Sta Cruz, Laguna was
shot at closed range by unknown lone gunman using unknown firearm.

3. After the incident the suspect immediately fled toward Pagsanjan, Laguna
direction using one color blue motorcycle Honda Wave without plate number as his get-
away vehicle.

4. Victim who sustained one gunshot wound at the back of his head was immediately
brought to Balubayan General Hospital, Sta Cruz, Laguna Laguna but was pronounced
dead on arrival by the attending physician, Dr Doray C Pulturero.

5. This office has requested the assistance from SOCO team of Laguna Provincial
Crime Laboratory Office, Bagumbayan, Sta Cruz, Laguna.

1. This pertains to the shooting incident happened on December 16, 2014 at about
10:00 in the evening at Kurikong KTV Bar located in Bagong Ilog, Sta Cruz, Laguna
shooting incident took place wherein the victim was identified as one Mario Egay Dugay, of
Bgy Sibol, Pila, Laguna.

2. Be informed that after the crime scene has been processed by SOCO team from
Laguna Provincial Crime Laboratory Office following pieces of evidence were
found/recovered from the crime scene as follows:

a. Three live ammunitions for cal 45 (marked MED 1, MED 2 and MED 3)
b. One empty shell of Caliber 45
c. One foot print (Left Foot, 5 inches in length and 2 cm width, marked as MED
d. One tire print/impression (Motorcycle Tire, 4 inches in length and 2 cm
width, marked as MED 5)
f. One bottle of Emperador Light (marked as MED 6)

3. This office has already created Special Investigation Task Team under STCE SO Nr
12-17-2014 composed of the following:

PSI DONATO DUCOT – Team Leader

SPO3 NONITO DONAIT – Chief Investigator
SPO2 ROTONI KOKOC – Investigator
PO3 LOLONG GULONG – Intelligence PNCO
PO2 GLORIA HAIST – WCPD PNCO

4. In-depth investigation still ongoing and any development relative thereto will be
forwarded to that office as soon as possible.
