KNOW ALL MEN BY THESE PRESENTS.

I, Merecita Cucal Obado of legal age, married, and a resident of Brgy. Sisilang, Bongabon
Nueva Ecija, after having been sworn in accordance with law, hereby deposes and states the following:

1. That I am the maternal aunt of Ma Rowena Cucal Marquez.

2. That I am present during her birth, on April 24, 1977.

3. That the name appearing on her certificate of live birth, Maria Rowena, and therein
date of birth, April 24, 1978 are both erroneous.

I hereby execute this affidavit to attest to the truth of the foregoing facts and for whatever legal
purpose this may serve.
