This is to inform you that Ms. Mae Ann B. Juri is hereby appointed and designated as Pollution
Control Officer (PCO) of the City Urban Development and Housing Office (CUDHO).
Currently, this office has an existing PCO, but due to numerous number of resettlement projects
being implemented by the City Government of Puerto Princesa across the sixty six(66)
Barangays of this City, additional PCO is badly needed.
In this accord, I would like to respectfully request for the accreditation and approval of our
newly appointed/designated Pollution Control Officer.
Attached herewith are the following documents in compliance with your requirements for
accreditation, to wit;
 PCO Accreditation Application Form
 Memorandum Order (Letter of Appointment)
 Certification of Employment
 Photocopy of Certificate of PCO Training
 Resume
 Diploma
 Notarized Affidavit of Joint Undertaking
 Professional Regulation Commission License
 Photocopy of Certificate of Managing Head Training
 Receipt of Processing Fee
