ACTIVITY COMPLETION REPORT

I. Identifying Information

ACTIVITY TITLE: Second Quarter Portfolio Day


BENEFICIARIES: Grade 10 – Rizza Adviser and Grade 10 – Rizza Parents

II. Location/Date

LOCATION OF BENEFICIARIES: Cahagnaan National High School


START DATE: March 6, 2021 @ 10:0 AM
END DATE: March 6, 2021 @ 11:30 AM

III. Summary/Narrative

The Second Quarter Portfolio Day was conducted on March 6, 2021. The Grade 10 classes
were scheduled to conduct the said activity from 10:00 to 11:30 in the morning at the Grade 10
Learning Resource Hub.
To ensure the quality of the teaching-learning process in these times of new normal in our
education system, teachers and parents need to talk about the progress of the learners. The parents
were informed to attend the activity to talk about the learners’ performance as well as resolve some
issues regarding the learners’ attitude and performance during the Second Quarter of the School Year.
The learners’ outputs were displayed for the parents to see. The learners’ progress and
performance were reflected in the students’ report cards and were showed to their parents. The
learners’ creativity were showcased in their outputs/projects.
The meeting ended at 11:30 in the morning after the parents signed the report cards of their
child/children.

IV. Actual Results

The parents learned about their children’s progress in the second quarter. They also heard
some of the feedbacks from the subject teachers about the learners’ performance. The portfolio day
was a success!

V. Major Problems Encountered

No major problems were encountered during the conduct of the Second Quarter Portfolio
Day. The teachers saw to it that health protocols were properly observed like distancing themselves
one meter apart from each other, applying alcohol and wearing the mask at all times. The checking of
the temperature was done by the school guard at the gate before the parents entered the campus.

VI. Insights/Recommendations

Based on the feedback from the parents, there should be ample time allotted for this activity.

were scheduled to conduct the said activity from 1:00 to 2:30 in the afternoon at the Grade 10
Learning Resource Hub.

PARENTS’ ORIENTATION ON THE IMPLEMENTATION


OF THE HOMEROOM GUIDANCE

ACTIVITY TITLE: Parents’ Orientation on the Implementation of the Homeroom

END DATE: October 13, 2021 @ 10:18 AM

III. Summary/Narrative

The Parents’ Orientation on the Implementation of the Homeroom Guidance was conducted
on October 13, 2021. The Grade 10 advisers were scheduled to conduct the said activity from 8:30 to
10:30 in the morning at the Cahagnaan National High School Multi-Purpose Covered Court.
Th Department of Education issues the implementation of the Homeroom guidance Program
for School Year 2021-202 which aims to establish guidelines and procedures. The CGP aims to guide
secondary level students as well as the parents in choosing career tracks that they intend to pursue
through informed career choices towards becoming productive and contributing individuals as stated
in RA 10533 otherwise known as the basic Education Act of 2013.
To ensure the quality of the teaching-learning process in these times of new normal in our
education system, teachers and parents need to talk about the progress of the learners including their
chosen career path. The parents were informed to attend the activity to talk about the learners’
preferences in the SHS strands and lso their choice of school.
Different topics about Homeroom Guidance were discussed by the Class Advisers while also
catering to the parents’ queries and suggestions. The grading system for the HGP modules was also
discussed as well as its components.
The orientation ended at 10:18 in the morning after the parents signed the report cards of their
child/children.

IV. Actual Results

The parents were enlightened about the different career paths available for their children.
Their questions about the scholarship grants if their children wishes to enroll at private high schools
were also answered. The orientation program was a success!
