Mad 4 My Dog Policies – Pet Sitting

I understand that I am solely responsible for any harm caused by my

dog(s) while my dog(s) is/are participating in walks, pet sitting or any
activities with Mad4MyDog Training, LLC, herein to be referred to as
M4MD.
I understand and agree that in releasing my dog(s) in M4MD’s care,
M4MD has relied upon my representation that my dog(s) is/are in good
health and have not harmed or shown aggressive or threatening behavior
towards any person or any other dog, or has disclosed any behavior to
M4MD as such.
While my dog(s) is/are in the care and custody of M4MD, if I am
unreachable in the event of an emergency, I hereby authorize M4MD, its
agents, and/or representatives to seek immediate veterinary care for my
dog. I understand that all costs in connection with veterinary, medical or
other treatment shall be my responsibility. In the event of a life-
threatening emergency requiring immediate veterinary care, M4MD is
insured and may be able to assist in the cost of that care. This will be evaluated and determined by M4MD on
a case by case basis.
I hereby release and agree to save and hold harmless, M4MD, its directors, officers, shareholders, employees,
assistants, members and agents from any and all liability, claims, suits, actions, loss, injury or damage of any
nature or kind, or for any liability, claims, suits, actions, loss, injury or damage which I or my dog(s) or my
property may sustain or which may be caused in any way by my dog(s). I specifically, without limitation, agree
to fully indemnify M4MD for any and all such liability, claims, suits, actions, losses, injury or damage, now and
in the future.
Although we carefully screen all applicants, occasionally we discover that this is not an appropriate service for
every dog. M4MD reserves the right to permanently remove a dog from our services at any time.
I certify that I have read and understand the rules and regulations set forth herein and that I have read and
understand this agreement. I agree to abide by the rules and regulations and accept all the terms, conditions
and statements of this agreement and confirm the truthfulness of the contents of the M4MD profile form
completed by me.
Please note: Full payment is due on or before the start date of the sitting job (with the exception of holiday
bookings - See policy below.) On or after the booked start date we cannot give a refund for any part of the
pet sitting job. Once the sitter is booked for your household, they cannot be booked again for those dates for
another household and that would be lost income for them. We thank you for your understanding!
Holiday season policy (includes Spring Break, Thanksgiving, and Christmas): Sitting fees for holiday bookings
must be paid in full at least 14 days prior to the start date of the job. If cancellation becomes necessary during
these holidays, it must be made 14 days prior to the booked start date of the sitting job in order to be eligible
for any refund. The 25% deposit is non-refundable at any time.

Informed Consent Agreement & Liability Waiver

Mad4MyDog Training LLC is referred to as “M4MD”, and the signed participant(s) will be referred to as
“Client(s)”. “M4MD Services” includes but is not limited to training classes, group sessions, play sessions,
play space rental, private training sessions, grooming, dog walking, & pet sitting. This signed statement
holds true now and at any point in the future, for any dog in my family now and in the future.

The Client has voluntarily participated in and/or requested M4MD Services, and certifies understanding
that there is always some unavoidable risk involved when working with animals. Dogs can be inherently
difficult to handle in various situations, and this can result in injury to the dog itself, other dogs, the Client,
the Client’s family members and/or friends, and third parties. The Client has had ample opportunity to
discuss any concerns of these risks with M4MD personnel, and has also informed M4MD personnel of
any behavioral concerns relating to the dog(s) they will be handling and/or involved with during M4MD
Services.

 The Client assumes full responsibility for the conduct of their dog and any resulting injury or
damage caused by dog. M4MD, and/or any host facility, cannot be held liable should an
altercation or injury occur between/to any person, animal, or property. The Client also accepts
and assumes, without reservation, all risks associated with participation in M4MD Services.
 The Client acknowledges the risk of the following, and M4MD is released of liability and/or
responsibility for any of the following:
o Injury (including disability and/or death) incurred by the Client’s self, the Client’s property,
the Client’s dog, and/or the Client’s family and/or friends
o Loss of or damage to personal property belonging to anyone present at any time
o Injury/Loss/Damage to any person, property, or animal caused by use of the M4MD
facility and/or equipment
o Injury/Loss/Damage caused to any person, property, or animal caused by use of any
technique, facility, or equipment having any connection whatsoever to M4MD
 The Client hereby waives, releases, discharges, and agrees not to sue and to indemnify, defend,
and hold harmless Mad4MyDog Training, LLC and its members, managers, agents, and
employees from any and all injuries, losses, claims, and damages to any person(s) of any nature
whatsoever, including claims arising from M4MD’s own negligence, and all costs associated
therewith, including attorney’s fees, court costs and consultation fees.
 This Informed Consent Agreement & Liability Waiver shall be legally binding on the Client.
Should the Client assert a claim to the contrary of what is agreed on here, the claiming party shall
be liable for all expenses (including attorney’s fees, court costs and consultation fees) incurred by
any and all parties named in the claim. If there is any dispute about or involving M4MD Services,
the Client agrees that the dispute shall be governed by the laws of the State of Indiana, USA,
without regard to conflict of law provisions and the Client agrees to exclusive personal jurisdiction
and venue in the state and federal courts of the United States located in the State of Indiana,
County of Monroe County.

This statement releases Mad4MyDog Training LLC of liability, and I am signing this acknowledgement of
risks voluntarily providing my informed consent of this agreement.

I have also been given the M4MD Policies Statement(s) for the M4MD Services I am utilizing, and provide
my informed consent to those policies as well.
