those years, we’ve received countless awards locally and internationally, recognizing our patented
products.

In behalf of the MAPECON PHILS., INC., the only nationwide pest control company operating
in Zamboanga City and our sincere desire to serve you, we are formally submitting a proposal to
undertake pest control services to your ongoing construction located at St. Mary Hills Malagutay,
Zamboanga City

SCOPE OF WORK:.

1. SOIL POISONING TREATMENT

1. Treatment of the flooring prior to cementing including the side wall surface;
2. Treatment of the whole interior soil surfaces areas after the grading and leveling of the soil in
the ground preparatory to pouring of concrete floor and along the outside perimeter of the
building after it had been fully leveled and ready for cementing;

QUOTATION AND TERMS OF PAYMENT:

Taking into consideration of the whole treatable areas with all our necessary chemicals,
supervision, labor, equipment, taxes and other materials needed in our services described above,

We quote you in the amount of:

Total Floor Area: 67.5 sqm x 150.00 = 10,125.00

MAPECON PHILIPPINES INC. Warrants that:

a. It is corporation, duly organized, validly existing and in good standing by virtue of the laws of the Philippines.
b. It has a necessary license from the National Committee on Urban Pest Control (NCUPC) to undertake pest control
services.
c. That only environment friendly DOST and international award winning MAPECON chemicals shall be used in pest
control services. Our chemicals are new Botanical insects growth Regulator which contains “No Insecticides” that
d. affect the nervous system so it very safe to health in accordance to proper handling and dosage.
e. It has a necessary capital and equipment to undertake the works contemplated in this proposal.
f. It has a full power and authority to execute, deliver and perform its obligation under this proposal.
g. There will be a Three (3) YEARS warranty period. In case of infestation, mapecon shall render retreatment without
additional cost as long as it covers within the warranty period.
