MINUTES OF THE MEETING

III. Call to Order: The meeting was called to order via Google Meet. It started at around
1:00 in the afternoon and was presided by Mr. Jhensen Iquin, Grade 6 teacher.

IV. Agenda

A. Completion of Requirements (discussed by Mr. Arnold Dacuma)

1. Academic Requirements
Advised the parents to make sure that the students are taking the test and submitting the output
on time to make sure that they have accomplished the tests and outputs before the graduation.

For transferees, make sure that the form 137 from the learner’s previous school and birth
certificate should be submitted to the adviser before graduation.

B. Attendance (discussed by Mrs. Normita Lozano)

Learners should attend their messenger classes for them to be updated on the test and output that
must be submitted and the schedule of submission.

C. Toga distribution and toga picture (discussed by Mrs. Fatima Claro)

Gave reminders to the parents/guardian of the learners who have not yet taken their toga picture.
- White toga and cap should be worn, the tussel of the cap should be on the right side.
- For those who will take their toga picture at home, should set up a green or any plain-
colored background (except red). They may borrow the toga from the adviser.
- They may also choose to have the toga picture taken at any studio.
- If they had the toga picture taken at school by Aljun, he shall be the one who will share
the soft copy to the teachers. Otherwise, the parents/guardians should send the copy of
the picture through the link that the advisers provided. The deadline of submission is on
June 2, 2022, the logo of the school will be used instead once they failed to submit the
copy of the picture. The advisers will also secure a letter of consent to be signed by the
parents for the use of the said picture as prescribed by the data privacy act.

D. Awards and Recognition (discussed by Mrs. Alice Santos)

Mam Alice opened her discussion with the DepEd Order no.18,s.2021 Interim Guidelines on
Giving of Awards and Recognition in Light of the Basic Education Learning Continuity Plan SY
2021-2022. Majority of the awards are suspended except for academic excellence. The academic
excellence is awarded to learners with gen. ave of 90 and a passing grade in all learning areas.
They may be awarded depending on the gen. ave of the learners- 98-100 (With Highest Honors),
95-97 (With High Honors) and 90-94 (With Honors). Mam Alice also tackled how the grade is
being computed and the weight of the written and performance tasks.

E. Distribution of Forms (discussed by Ms. Sarah Jane Pelos)

Report cards will be distributed today (May 23, 2022), the card of learners with complete grades
will be released while those with blank(s) will be put on hold until the learner accomplished what
they lack.

The end of 4th quarter is on June 24, 2022. On July 15, 2022, the hard copy of the report card,
diploma and toga picture will be distributed.

F. Open Forum

The following are the concerns of the parents and guardians and responses of the teacher (s):

 Release of form 137 – no exact date of its release as the request for the form must be
made first by the school where the learner has transferred.
 Hard copy of birth certificate – must be submitted before graduation.
 Learners who will not receive (on hold) their report card should get in touch with their
respective subject-teacher to accomplish what they lack.
 Request for gmc, should be made via the link. The release of the gmc is on every Fridays.
 For learners who are listed as modular may opt not to attend the messenger classes as
long as they submit the accomplished activity sheets.
 There is no final decision yet as to whether the graduation will be held via face-to-face or
virtual.
 First communion, parents/guardians should coordinate with Bro. Oscar. May 28 is the
schedule of the communion.

V. Adjournment: The meeting was adjourned at around 2:30 in the afternoon.
