NOTICE OF TERMINATION OF ADMISSION TO THE CDS UNDER REGULATION 15.10.1

OF THE CDC REGULATIONS

I/we hereby give notice under Regulation 15.10.1 of the Central Depository Company of Pakistan
Limited Regulations (“CDC Regulations”) for termination of my/our admission to the Central
Depository System (“CDS”) in respect of my/our CDS Participant Account (ID # _________).

I/we hereby represent to CDC that in compliance with Regulation 15.10.2 of the CDC
Regulations, I/we shall complete all the formalities of the Procedures within 40 Business Days
from the date of CDC’s notice (“the Compliance Period”) to be issued pursuant to this notice,
failing which I/we shall be liable to pay Rs. 100 per day to the CDC from the first day after the
expiry of the Compliance Period.
