Schools Division Superintendent ChiefCID and SGOD Education Program Supervisors Public Schools District Supervisor Public & Private Elementary & Secondary School Heads All others concerned eal -fentttiio Pasheh, ces0v From Schools Division Superintendent ga, Subject : CREATION OF DIVISION CHECKING COMMITTEE Date : February 13, 2019 Pursuant to DepEd Order No, 11, s. 2018, Guidelines on the Preparation and Checking of School Forms, the Division Checking Committee (DCC) is created to facilitate the timely, consistent and quality checking of forms. The composition is as follows: Chairman Chief, Curriculum Implementation Division (CID) Vice Chairman: Chief, School Governance and Operations (SGOD) Public Schools District Supervisors (PSDS) Members + Education Program Supervisors (CID) to be identified by the Chair Senior Program Specialist for planning and Research (SGOD) Division Planning Officer Other Division or Schoo! Personnel to be identified by the Chair and vice-Chair 1. The roles and functions of the CID shall focus on the areas of enrolment eligibility and assessment which includes promotion/retention or acceleration to ensure that the most recent policies are strictly observed while the SGO shall validate the enrolment count, number of school leavers (dropped out), number of learners who transferred in/out, were promoted/retained and enrolment-related indicators. The SGOD shall also conduct spot checks or hhead counts of learners vis-a-vis the SF1 and SF5 as necessary. The division System Administrator who is the Planning Officer and the Senior Education Program Specialist for Planning and Research shall validate the presented SF and SF5 with the actual online record of school in the US. 2. The division checking committee is expected to lead as well as coordinate with the School Checking committee in the preparation and filling up of templates to ensure timely submission. 3.
