TO ALL CONCERNED OFFICIALS AND EMPLOYEES

SUBJECT Reiteration on the Guidelines of SECPA Requests of


PSA Reaular and Contract of Service Workers (COSVV's) at
the CRS Outlets of Tacloban City and Catbaloaan City

DATE 1 2 April 2021

Pursuant to RA 10173 (Data Privacy Act of 2012) and RA 9485 (Anti Red
Tape Act of 2007) and for uniform instruction on Civil Registry Documents
SECPA requests of PSA Regular and Contract of Service Workers (COSW's)
at CRS Outlets in Tacloban City and Catbalogan City, the following
guidelines must be observed for proper monitoring and documentation:

1. PSA employees (regular/COSW's/Associates) are required to lodge


their requests of Civil Registry documents in SECPA copies thru
Division/PO Designated Focal Person:

CRASD Focal Person -Ms. Marian C. Claridad

SOCD Focal Person - Ms. Menjie P. Rapafia

PO -(Designated Focal Person)

The Focal Person shall be the sole person to transact 'with


CRS Team Lead - Mila R. Hermo or Gizelle M. Advincula at the
MCR's Lounge Area and must record their requests in the logbook with
the required information.

2. All SECPA requests of PSA employees must be compliant with the


requirements like Authorization Letter or Special Power of Attorney and
valid lD if the requester of the document/s is the authorized
representative.
3. SECPA requests of PSA Employees must be limited to t`^ro (2) requests per
day which will be processed within three (3) days from receipt of the
requests. The cut-off time for the submission of the request shall be at
2:00 PM only during working days.

4. SECPA requests are not accumulated. Meaning if no requests for the day it
will not add up to the next days' requests. Example: Monday- 3 requests;
Tuesday-no request: Wednesday- 1 request; Thursday- 2 requests; Friday- 3
requests. This example is not allowed for there should only be 2 requests
during the day.

5. SECPA requests of Contract of Service Workers (COSW's) will only be


allowed for emergency cases but limited to one (1) request per day.

6. In no case shall SECPA requests be directly transacted at the


CRS area/CRS Associates for they are not allowed to entertain special
requests from clients and PSA employees. This is in compliance with UNISYS
Memorandum dated 10 July 2019, Guidelines Against Providing Special
Treatment and Non-Standard Assistance to Requesters, and
UNISYS Memorandum dated 28 January 2021, Prohibiting UNISYS Field
Office Personnel and FOP Team Leads from Dealing Directly with the
Local Civil Registry Offices.

7. All SECPA requests shall be monitored as to the full compliance of every


concerned employee of PSA for proper implementation and facilitation of
issuance of civil registry documents.

8. A box will be provided by CRS where requested documents will be


stored/kept. Non-CRS personnel are not allowed to transact business
inside the CRS Outlet Processing Area.

9. Outlet Supervisor shall ensure that the monitoring of requests is being


disseminated every end of the month during the flag ceremony.

This Office Memorandum shall take effect immediately.

Please be guided accordingly.
