AFFIDAVIT OF ADMISSION OF PATERNITY / RECOGNITION

AND TO ALLOW THE USE OF SURNAME

I, Carl Joseph Martonia, Filipino, single, legal age and a resident Serenis North
Subd, Yati, Liloan, Cebu after having been sworn to in accordance with law, do
hereby declare that;

1. I am the biological father of Cristine Mae Nayre.

2. The mother of Cristine Mae Nayre is Lorrie Mae Nayre.
3. Cristine Mae Nayre was born December 10, 2017 at Saint Anthony Mother
and Child Hospital Basak, Pardo. Her Birth Certificate is duly registered with
Registry No. 2017 35959.
4. Lorrie Mae Nayre and I do not have legal impediment to marry each other
when Cristine Mae Nayre was conceived.
5. Cristine Mae Nayre has been living with me since she was a baby. She is now
4 years old.
6. That being the father of Cristine Mae Nayre, I hereby acknowledge and
recognize her as my child and legal heir;
7. That I am allowing my child to use my surname “Martonia” to be her surname
in her Certificate of Live Birth, school records and all public documents
8. That I execute this affidavit to attest to the truth of the above facts in
compliance with the provisions of Republic Act No. 9255.
9. I hereby certify that the statements made herein are true and correct to the
best of my knowledge and belief;
10. This Affidavit is explained to me and I understand its content.
