Re: Notice of Withdrawal of Shares of Stocks

Dear ______________:

I write to you in relation to our shares of stocks with ______________), with a

total of One Thousand (1000) Common Shares of Stocks with par value of
P100 per share.

I wish to withdraw all my shares with ____________ due to personal reasons. If

anyone from the existing stockholders wishes to buy the said shares, I am
willing to sell said shares based on the current valuation.

Also, I have not encashed the last cheque that I received from ___________ as
part of my dividends which was distributed last December 2021 due to the
incorrect spelling of my first name. Said cheque had already become stale due
to the lapse of the six (6) months period when it should have been deposited.
Hence, may I request for the issuance of another cheque in lieu of the old
cheque issued to me.

I am also requesting for the full accounting of all income of __________ that had
accrue by reason of all its investments and products offered to it clients or the
delivery of the total equivalent income of my shares of stocks since the start of
my venture with __________.
