I, NORBERTA E. SURWELOS, of legal age, single, Filipino and a resident

of Brgy. Baclaran, Balayan, Batangas after having been sworn in accordance with
law, hereby depose and say;

1. That I am the registered owner of the land located at Brgy. Sampaga,

Balayan, Batangas;

2. That the said title is not in the custody of Ofelia B. Magahis;

3. That I am executing this affidavit to attest to the truth of the foregoing

statements.
