Meeting called to order at 8 a.m. by 10-St. Isidore Adviser, Mr. Jaypee C. Felizardo

I. SET A and SET B of modules

The adviser explained why there is need to have Set A and Set B for module
distribution. Present the list of Set A and Set B to the students and parents for them
to be informed. Explained that they need to accomplish the given modules and
submit it on time because the students who received Set A will use Set B for the
following week and vice versa.
II. Module Bag
The adviser encouraged the parents to take good care of the module bag
III. Importance of Modules
The adviser explained the importance of modular learning in this time of pandemic.
Remind the students and parents of the following:
a. Please use the modules properly because other students will also use the
same module.
b. They are NOT allowed to write their answers on the modules.
c. They are NOT allowed to tear the pages of the modules.
d. They are NOT allowed to reproduce the copy of the modules.
e. Do NOT lose the modules
IV. Submission of Answer sheets
The teacher will explain that the students can write their answers in bond paper,
yellow paper or intermediate pad. Advise the students to write the following on their
paper:
a. Complete Name
b. Grade and Section
c. Module Number
d. Subject
The penmanship must be readable and do not place the answer sheets between the
pages of the modules.
V. Schedule of module distribution and retrieval
The teacher informed the parents about the schedule of retrieval and distribution of
modules. The teacher reminded the parents to observe and practice health protocols
such as proper wearing of facemask and social distancing, bring their own ballpen,
pay attention to the signages of school, and only 18-59 years old are allowed to
receive and submit the modules.
VI. Contact Tracing Form
Remind the parents to bring Contact Tracing Form written in ¼ piece of intermediate
pad every module retrieval and distribution. Place their contact tracing form in the
drop box.

VII. Soft copy of modules

Parents need to submit the letter of request for the distant learners who needed soft
copy of the modules. They are NOT allowed to reproduce the copy of the modules.

VIII. Homeroom Guidance

Advised the students to answer the Homeroom Guidance and submit it together with
their answer sheets.

IX. Constant communication of Teachers, Students and Parents

Remind the parents and students to have constant communication with the adviser
and subject teacher. Please be reminded to respect the time of the teachers when
asking inquiries.
