I am writing to refer Miss Foulkes, a 34-year-old freelance journalist, whose features are suggestive of
inflammatory bowel disease. Your urgent assessment and management would be highly appreciated.
Today, Miss Foulkes presented with the complaints of bloody stool, severe intermittent stomach cramps,
diarrhoea lasting for more than 7 days at a time and unusual fatigue. However, she reports no obvious
triggers. Her physical examination was unremarkable except for mild abdominal tenderness. Her
investigation reports revealed anaemia on FBC, raised levels of ESR, CRP and faecal calprotectin. Please
note, test for coeliac disease was negative. Therefore, she was advised for smoking cessation and dietary
modification.
Miss Foulkes is single and lives alone. She smokes 10-12 cigarettes per day. Her medical history is
significant for digestive problems since childhood, which included self-diagnosed IBS since 2009 and an
episode of viral gastroenteritis in 2017. Despite the conditions causing anxiety, she is unwilling to take
any medication.
I believe Miss Foulkes has been suffering from inflammatory bowel disease and needs your urgent
attention. I would be grateful if you could evaluate her promptly to establish a definitive diagnosis.

Today, Miss Foulkes visited me with the complaints of bloody stool, severe stomach cramps with
diarrhea lasted for a week and fatigue without any definite triggers. She also stated disruption of her daily
life. Her physical examinations including rectal examination were normal except for mild tenderness on
her abdomen without any palpable mass. Her investigation results revealed anemia as well as elevated
level of ESR(28mm/hr), faceal calprotectin (38mg/l) and CRP(55mg/l). Please note, the test for Coeliac
disease was negative. Hence, she was advised for smoking cessation and Mediterranean diet.
Miss Foulkes, who travels frequently, is single and smokes 10-12 cigarettes every day. Regarding her
medical history, she has had digestive problems since her childhood and had diagnosed herself as IBS for
which she had tried a variety of diet plans including reduced consumption of dairy products. In addition,
she had a history of viral gastroenteritis in 2017. Besides, she is worried of having Crohn’s disease.
I believe Miss Foulkes has been suffering from IBD. It would be grateful if you kindly evaluate her and
diagnose her condition accordingly.
