Sub: - Safety Violation at Fabrication Yard

As on 24.11.2015 walk through was held at 10.30am with the team of PLL, IHI,
VTV, & AFCON. At the time of walk through two workers were found sleeping at
area-1 at the time of 11.10 am. (Fabrication Yard) found sitting near the stacked
nickel plates. This was serious violation so management decided to impose
hundread rupees penalty per head. (100 /-). The name of worker is Ishteyak (IHI-
VTV-086) helper & Ankit Kumar (IHI-VTV-1274) helper.

Counseling was imparted & assurance taken from this worker that such type
violation will not happened again in future.

We have extra one no of Walkie Talky to meet for any exigencies.

We have instructed to all our concerned Engineers, Supervisors and HSE

personnel to re charge properly prior to use of the Walkie Talkies for proper usage
during communication. Also we review Walkie Talkies on regular basis to
strengthen our communication.

As on 23.02.2016 walk through was held at 10.30 am with the team of PLL, IHI,
VTV, & AFCON. At the time of walk through one worker was found sleeping on
pellet at the front of T-106, where IHI container were kept at the time of 11.10
am.. This was serious violation so management decided to impose five hundred
rupees penalty, (500 /-). The name of worker is All Jhan (IHI-VTV-928) rigger.

Counseling was imparted & assurance taken from this worker that such type
violation will not happened again in future.
