I. PUPIL DEVELOPMENT
1. Conducted the following tests:

1.4 National Achievement Test – March 8, 2012
1.5 Division Achievement Test – March 22, 2012
1.6 Post-test – March 2012

2.2 The ESIHAS Digital Film Festival
3. Participated in the following programs and activities:
3.1 Nutrition Month
3.2 Buwan ng Wika
3.3 Reading Month
3.4 Physical Fitness Day

II. STAFF DEVELOPMENT


1. Participated in the Division Seminar Workshop on Documentary and Film Making.
2. Served as Computer Class teacher for Grades V & VI pupils.
3. Served as Editor in Santa Rosa District Production Team.
4. Served as School Nutrition and ICT Chairman.
5. Served as Trainer in the School Based Journalism Training Workshop.
6. Served as Evaluator during the School Reading Month Celebration.
7. Attended the district meeting of the School Nutrition Chairman at Santa Rosa Central School on
June 2011.
8. Chaired the School Nutrition Month Celebration.
9. Attended the division meeting of ICT chairman held last July at the Library Hub, Cabanatuan
City.
10. Participated in the 6th Esihas Digital Festival.
11. Participated in the ESIHAS Float Parade and Exhibit last November 13, 2011.
12. Attended and received awards in the Awards Night of the ESIHAS.
13. Participated in School-Based Training Seminar-Workshop on E-Classroom & Interactive White
Board Application on February 23 & 24, 2012.
14. Participated in the 2012 Multimedia Learning Resources Evaluation on March 9, 2012.
15. Participated in the conduct of the School’s Clean-up Drive Activity.
16. Participated in the School Earthquake and Fire Drill conducted by the Municipal Fire
Department officials.
17. Attended school meetings.
18. Participated in SLAC Session and In-Service Training for Teachers.

19. Elected as Business Manager of the SGES Teachers’ League Officers.


20. Served as Room Examiner in the 2011-2012 National Achievement Test for Grade Six at St.
Christopher Montessori School.
21. Participated in the ALS National Tree Planting, 2012 Lingap Kalikasan Program on March 30,
2012 at Brgy. Tagpos, Santa Rosa, Nueva Ecija.
III. CURRICULUM DEVELOPMENT
1. Prepared valid and reliable formative and summative test.
2. Prepared questions for the test item bank.
3. Prepared low-cost devices suited for the grade in Science, MSEP and ICT.
4. Integrated Health & Nutrition in the different learning areas being taught.
5. Conducted and prepared training/seminar and prepare training design. (e.g. Symposium in
Promoting good Health)
6. Used Information and Communications Technology (ICT) resources for planning and designing
teaching learning activities.
IV. PHYSICAL DEVELOPMENT
1. Put up plants in the facade of the computer laboratory.
2. Maintained the cleanliness and orderliness of the computer laboratory and area assignment.
3. Put up water line in the comfort room of the computer laboratory.
V. ANCILLARY SERVICES
1. Put up new instructional materials in computer laboratory.
2. Followed the testing program.
3. Participated in the community Clean and Green program.
4. Accumulated reports and records.
5. Monitored and conducted supplementary feeding given to pupils below normal.
6. Monitored curriculum integration of different health & nutrition programs/ projects.
7. Prepared different reports needed to be accomplished on time.
8. Consolidated the Nutritional Status Report of Teachers and pupils from Grades I-VI Baseline
and Endline.
9. Encoded pre and post-test results of Phil-IRI (English and Filipino) from grades I-VI,
Baseline/Endline Nutritional Status report of pupils from grades I-VI, Teachers’ NCBTS/TSNA,
and updated online the PAG-IBIG membership of teachers.
10. Distributed a pair of toothbrush and toothpaste to all Grade I pupils sponsored by the Division.
11. Participated in the following activities:
a. Brigada Eskwela
b. Alay Lakad Campaign
c. DepEd Founding Anniversary
d. Division and District Education Summit
e. World Teachers’ Day Celebration
