I. BACKGROUND INFORMATION

The proposed category __ Sanitary Landfill site is seated on the following geographic
coordinates: 14.832167 North Latitude and 121.139301 East Longitude covering an area of
28.38 hectares and is accessible via Commonwealth Ave/Don Mariano Marcos Ave and Manila
Gravel Pit Rd/Payatas Rd and the Commonwealth Ave/Don Mariano Marcos Ave to CSJDM
or Mindanao Avenue and Quirino Hwy to Welcome Rd in CSJDM.

the proposed SLF will cater the waste from municipalities of Bulacan, nearby cities and
other municipalities within the close vicinity, and will have an estimated waste handling
capacity of ______ tons per day (TPD).

This site assessment is in accordance with the site identification criteria stipulated in the
Republic Act 9003, and the National Solid Waste Management Commission Resolution No.64,
series of 2013 which mandates this Office to provide technical assistance in conducting
environmental site assessment, identification and selection for proposed waste management
facilities.

II. FINDINGS AND OBSERVATION

1. The area generally lies FLAT/SLOPPING terrain and is surrounded by various vegetation
such as _____, __________________.
2. In terms of the compatibility of the proposed project with the existing land use plan, the
area is currently zoned as __________ land. Favorable clearances from the Department
of Agriculture (DA) and Department of Agrarian Reform (DAR) as well as Municipal
Resolution regarding the use of the said land shall be secured;
3. Based on the rain-induced landslide and flood susceptibility maps, and the results and
findings, the area has low and medium landslide and flood susceptibility rating. However,
according to our INITIAL findings “Majority of the area of the identified proposed site
is not susceptible to flooding except those areas near active river channels which be
inundated duringoccurrences of heavy rains thus experiences moderate to high”
4. Aerial and Table survey shows that the proposed project site is partly part from the
NIPAS and 525.500 away from the CRITICAL WATERSHED.
5. According to the result of HazardHunterPH, the nearest Active Fault is a segment of
West Valley Fault situated about 857 meter and 1 km from the proposed site
