I am Christian Kim M. Caguioa, with legal age and a bonafide resident of Barangay
22 Manzon, San Carlos City, Pangasinan. I am presently employed as a regular
permanent Senior High School Teacher at Tandoc National High School. I am
assigned coordinator of Technical Vocational Livelihood (TVL) track and I am
currently teaching TVL subjects offered in our school.

National Certificate II so that I would be more qualified and competitive in teaching
