Lot 8 Blk 8 San Jose St. Teresa Homes, Tibag, Tarlac City, Tarlac do hereby
appoint MRS. EDNA C. GARCIA, of legal age, married with residence at Maasin,
San Clemente, Tarlac to be my true and lawful attorney-in-fact for me and in my
name, place and stead, to do and perform the following acts:

1. To conduct all activities in the processing of my government

requirements in the exercise of my profession particularly in relation to
government agencies including BIR, DTI and other local government
unit requirements.

2. To fill out forms and sign, follow up said the documents.

3. I grant her permission to process and claim any other documents on my

behalf as if I am the one acting.

4. The scope of this authorization letter includes identity proofs for

identification purposes.
