g. the discounts we offer and the methodology for their


application for this Contract are:

h. our Bid shall be valid for a period of days after


the date fixed for the opening of bids in accordance with
the Bidding Documents, and it shall remain binding upon
us and may be accepted by you at any time before the
expiration of that period;

i. if our Bid is accepted and we receive from you a Notice


of Award, we commit, within ten
(10) calendar days after our receipt of the said
Notice, (1) to submit to you the required
Performance Security and other documents prescribed
in the Bidding Documents, and (2) to sign the
Contract Agreement;

j. we understand that, if the contract is awarded to


us, this Bid, together with your written acceptance
thereof through your Notice of Award, shall
constitute a binding contract between us, until a
formal Contract Agreement is prepared and executed;

k. we understand that you are not bound to accept the


Lowest Calculated Bid or any other Bid that you may
receive; and

l. we acknowledge that failure to sign each page of


this Form of Bid and the accomplished Bill of
Quantities shall be a ground for the rejection of
our Bid.

2. We submit the enclosed Affidavits of Commitment to Work on the contract of

3. We ensure that the abovementioned personnel shall employ their best care,
skill, and ability in performing the duties of their respective positions
in accordance with the provisions of the contract, including the Conditions
of Contract, Specifications, and Drawings, and that they shall be
personally present in the jobsite during the period of their assignment in
the contract.

4. In the event that we choose to replace any of the abovementioned key


personnel, we shall submit to you in writing at least fourteen (14) days
before making the replacement, for your approval, the name and bio-data of
the proposed replacement whose experience shall equal to or better than the
person to be replaced.

5. We understand that any violation of the above-stated conditions shall be


sufficient ground for us to be disqualified from this Contract and future
biddings of the DPWH.

CONSTRUCTION POLICY
I consider my employees as important assets that will strive to
protect and preserve, together with the property, plant and
equipment I work with and the environment we operate in. Thus, I am
committed to complying the requirements on Occupation Safety and
Health Standards.

effect of my operation on my safety, Health and Environment. I am
committed to both continues improvements in my workers, clients and
the communities, in which I live and work.

CONSTRUCTION SAFETY AND HEALTH PROGRAM BY CONTRACTOR


BASED ON SECTION 1.B 1.39 (DOLE) OCCUPATION SAFETY AND
HEALTH STANDARS
I. SAFETY PROGRAM
1. Contractor provides skull guards, raincoats, working shoes and
boots to employees who are assigned to hazardous area.
2. Wires, nails, bolts and other pointed objects should be
eradicated in working areas to avoid possible
injuries/accidents.
3. Operator, drivers and other employees who handle equipment
must thoroughly check their equipment, lubricate and handle
them properly and should be cautious, extra careful at all
times to avoid accidents while on duty.

4. Employees are advised on the use of cigarettes, candles and


other flammable materials to avoid the occurrence of fire.
5. Seat belts are provided in every truck/vehicle that is used in
the project sites.
6. Fire extinguishers are being placed in equipment such as fuel
truck.

II. HEALTH PROGRAM


1. Upon entrance, employee are automatically
become members of SSS and phil. Health Corporation.
2. Medicine cabinets which contains over the
counter drugs and other first aid supplies are ready for use in
case of sickness or accident occur.
3. Employees are being lectured once in a while or
briefed on personal hygiene, numbers if children one family would
have, to be able to provide the family needs on food, clothing
and shelter.
4. Strictly no drinking of liquor during working
hours to ensure safety and for health purposes.

CONSTRUCTION METHOD

Facilities for the Engineers


The Contractor shall provide and maintain field offices
and testing laboratories, including all the necessary
electricity, water, drainage and telephone services for the
use of the Engineer and his staff. The offices shall have at
least the floor area prescribed on plans and shall contain the
equipment, supplies and furnishing specified in the contract.

Provision Of 4x4 Pick Up Type Service For The Engineer


Vehicle shall maintain the vehicle in the first class
condition and shall be supplied with appropriate fuel and
lubricants at all times.
It shall provide equivalent substitute vehicles during
any period when the specified vehicles are taken out of
service for maintenance, repair or any other reason. Unless
otherwise specified, the vehicle shall at the end of the
contract become the property of the Government.

Operation Maintenance Of 4x4 Pick Up Type Vehicle For The


Engineers
Vehicle shall maintain the vehicle in the first class
condition and shall be supplied with appropriate fuel and
lubricants at all times.
It shall provide equivalent substitute vehicles during
any period when the specified vehicles are taken out of
service for maintenance, repair or any other reason. Unless
otherwise specified, the vehicle shall at the end of the
contract become the property of the Government.
There shall be employ permanently on the site a fully
trained Medical Aide who shall be engaged solely from medical
duties.

Miscellaneous Survey and Staking


The contractor shall at all times during the duration of
the contract provide for the use of the Engineer all
equipment, instruments, and apparatus, all information and
records and qualified chainmen and laborers required by the
Engineer for inspecting and measuring the Works. Such
equipment, instrument and apparatus shall include those listed
in the special provisions.

Project Billboard/ Signboard


This item consist of providing a standard DPWH layout
printed on tarpaulin with wooden frame to be located at the
front of the project site.

Occupational Safety and Health Program


Provide and maintain throughout the duration of the
contract, a medical room together with all necessary supplies
to be sited in the main area.

There shall be employ permanently on the site a fully


trained Medical Aide who shall be engaged solely from medical

Traffic Management
The contractor must observe traffic management during the
construction specially busy/traffic street or roads. He/She
provide flag men, portable traffic light or other traffic
management apparatus or as per detail.

Mobilization/ Demobilization
This item consist of mobilization of equipment and
materials needed during the implementation of project and
demobilization of the equipment and materials used after the
project.

Clearing And Grubbing


This item shall consist of the removal wholly or in part,
and satisfactory disposal of old pavements which are not
designated or permitted to remain, except for the obstruction
to be removed and disposed off under other items in the
contract. It shall also include the salvaging of designated
materials, backfilling the resulting trenches, holes and pits.

REMOVAL OF EXISTING CONCRETE PAVEMENT


This item shall consist of the removal wholly or in part,
and satisfactory disposal of old pavements which are not
designated or permitted to remain, except for the obstruction
to be removed and disposed off under other items in the
contract. It shall also include the salvaging of designated
materials, backfilling the resulting trenches, holes and pits.

Surplus Rock Excavation(Soft)


This item shall consist of the removal wholly or in part,
and satisfactory disposal of old pavements which are not
designated or permitted to remain, except for the obstruction
to be removed and disposed off under other items in the
contract. It shall also include the salvaging of designated
materials, backfilling the resulting trenches, holes and pits.

Aggregate Base Course


The existing surface shall be graded and finished. The
aggregate sub base material shall be placed at a uniform
mixture on a prepared sub grade in a quantity which will
provide the required compacted thickness.

Bituminous Tack Coat


This item shall consist of preparing and treating an
existing bituminous or cement concrete surface with bituminous
material in accordance with the plans and specifications,
preparatory to the construction of a bituminous surface
course.

Bituminous Surface Course


This item shall consist of constructing a Bituminous
Concrete Surface Course composed of aggregates, mineral filler

and bituminous material mixed in a central plant, constructed


and laid hot on the prepared base in accordance with this
specification and in conformity with lines, grades, thickness
and typical cross-section shown on the Plans.

Portland Cement Concrete Pavement


The engineer conforming to the design mix approved by the
materials engineer shall recommend concrete mixture. Cement
and aggregates shall be mixed by a motorized concrete mixer
containing 25 liters of water per bag of 40 kg. Portland
cement. Placing of concrete mixture shall be conveyed by GI
flume or concrete cart. A concrete vibrator shall be used to
let the concrete mixture settle properly and avoid the
appearance of voids. Weakened plane joint shall be installed
at every 4.5 m. Curing shall take place for a period of 72
hours by means of pounding.

EROSION CONTROL MATS


This item shall consist of furnishing and placing erosion
control mats, roving, and cellular confinement systems
installed at the locations designated for ditch and slope
protection, and stabilization, in accordance with this
specification and in conformity with the lines, grades,
dimensions, and arrangements shown on tt-Ie plans or as
directed by the Engineer.

PERMANENT GROUND ANCHORS


This Item shall consist of designing, furnishing, installing,
testing and stressing permanent cement-grouted ground anchors
in accordance with the plans, these specifications, and the
special provision.

HYDROSEEDING
This item covers installation of coconut coir fiber made
into geonets such as coco-nets/mats, coco-Iogs/fascines,
coco-twines and coco peat as bioengineering materials for
controlling soil erosion and slope stabilization in accordance
with the cross section shown on the Plans or as directed by
the Engineer.

Active Wire Mesh System


Rockfall netting shall be made from flexible steel woven wire
mesh coated with zinc and/or zinc alloy or a combination of
zinc and plastic coating. The mesh can be used in combination
with steel cables interwoven in the mesh during the production
as a composite material. The wire mesh shall be dimensionally
stable and able to retain its geometry under manufacture,
transport, and installation. The contractor shall submit a
manufacturer's certification that the wire mesh supplied met
the criteria set when the wire mesh was approved, measured in
full accordance with all test methods and as required in this
specification. In case of dispute over validity of values, the
Engineer shall require the contractor to supply data from an
approved laboratory to support the values submitted, at the
contractor's cost.

Reflectorized Thermoplastic pavement Markings


The pigment, beads and filler shall be uniformly
dispersed in the resin. The materials shall be free from
skins, dirt and foreign objects. The materials shall not exude
fumes which are toxic, obnoxious or injurious to persons or
property when heated during application.
Center lines, lane lines and edge lines shall be applied
by approved mechanical means and shall be laid in regular
alignment.
