TERMS OF AGREEMENT

KNOW ALL MEN BY THESE PRESENTS

This TERMS OF AGREEMENT is made and entered into this (Date) in Calamba City Laguna Province
by and between:

BCNC WATER REFILLING STATION, a private business duly organized under the law of Philippines registered
under the Department of Trade and Industry. With business address at Blk. 43 Lt. 12 St. Martin St. MCDC
Kapayapaan Viille, Canlubang Calamba City Laguna. Represented herein by Mr. Benjie Buenaventura Philippine
National of legal age. Herein referred to as “OWNER’’.

,hereinafter referred to as “BORROWER”.

WITNESSETH

Whereas the OWNER, owns a Mitsutech Hot and Cold water dispenser, to lend to the BORROWER, under the
brand “BCNC Waters” to be supplied by the OWNER.

Whereas the BORROWER accepts the BORROWED (1) Mitsutech Hot and Cold water dispenser which will be
used exclusively for “BCNC Waters” brand supplied by the OWNER.

The parties hereto, as they hereby agree as follows

1. TERM
1.1 CONTRACT PERIOD : Valid as long as mutually agreed by both parties.
The term starts from (mm/dd/yy)
1.2 The Borrower must maintain a minimum purchase of 20 containers of water refill per week and to
pay every Monday of each week the accumulated order of water refill in order to retain the Mitsutech
hot and cold water dispenser.

2. RIGHTS OF THE BORROWED GOODS

The OWNER, has a legal ownership of the Mitsutech Hot and Cold water dispenser. During the
borrowing period, the BORROWER has no right to lease, lend, sell, transfer or mortgage the
borrowed Mitsutech Hot and Cold water dispenser to any third party or to dispose the Mitsutech
Hot and Cold water dispenser in any other way. Otherwise, the OWNER has right to withdraw the
borrowed Mitsutech Hot and Cold water dispenser, as well as terminate the contract at any time.

3. USAGE
1. The BORROWER is obliged during the borrowing period to keep custody, maintain and use
the Mitsutech Hot and Cold water dispenser for which it is intended. The BORROWER is
responsible for any loss or damage of containers and the hot and cold water dispenser
due to mishandling and negligence, and must pay the equivalent cost.
2. The OWNER warrants that the Mitsutech Hot and Cold water dispenser are in good condition.
