LETTER OF PARENTAL CONSENT AND WAIVER

For Limited Face-to-Face Classes, SY 2022-2023

Dear Ma’am/Sir,
Greetings of Peace in Christ Jesus!
The HOLY CHILD College of Davao – College Department is now preparing for the
possible conduct of Limited Face-to-Face classes among its students in School Year
2022-2023. With this, the necessary documents as well as the needed facilities and
equipment are now being prepared by our personnel. This endeavor is also based on the
survey conducted among the college students wherein 48.1% or majority of the college
students who responded to the survey said that they prefer face-to-face classes as the
learning modality for the next school year. Additionally, majority of the college students
are already fully vaccinated which implies that these vaccinated students are qualified to
be engaged into limited face-to-face classes and activities inside the campus.
If this limited face-to-face classes will be implemented in SY 2022-2023, only the subjects
that requires practice or face-to-face interaction will held face-to-face. Examples of these
subjects are those having laboratory practices. On the other hand, those subjects which
are more on theories and do not necessarily require a face-to-face engagement will still
be conducted online. Additionally, specific schedules on face-to-face meetings will be
provided by the program heads for you to know the specific days that your child needs to
report to school.
In relation to this, we would like to seek your consent/permission to allow your child to
attend the classes that will be conducted in a limited face-to-face set-up. Rest assured
that the face-to-face classes will be conducted in an organized manner and will be
supervised not just by the instructor but as well as the program heads, health officers and
other authorized personnel. Furthermore, our institution prepared guidelines on COVID-
19 health protocols and contingency plan for COVID-19 resurgences and we can

guarantee that these protocols will be followed by the students and personnel while inside
the campus.
Thus, all precautions will be taken to safeguard all the students and personnel during the
conduct of limited face-to-face classes. However, the HOLY CHILD College of Davao
cannot guarantee your child’s ABSOLUTE safety from any potential risks or source of
infection. Please be reminded as well that your child’s participation in this endeavor is
voluntary and only those vaccinated students will be allowed to take classes inside the
campus.
Please read further the attached waiver and affix your signature once you have decided
to allow your child to attend the limited face-to-face classes next school year.

to attend the limited face-to-face classes of HOLY CHILD College of Davao – College
Department to commence on August 2022 until further notice. I hereby acknowledge the
benefits that my son/daughter will derive from this endeavour with understanding that due
care and precaution will be observed to ensure the comfort and safety of students and
personnel during the conduct of face-to-face classes inside the campus.
Furthermore, I clearly understood all the information that has been relayed to us during
the conduct of Limited F2F Classes Parents’ Consultation on May 12, 2022 and I also
acknowledge the potential health risks of allowing my child to attend the said face-to-face
classes. Despite such risks, I allow my child to take and attend the classes that will be
conducted in a limited face-to-face set-up.
I do hereby confirm and declare that I am allowing my child to attend the said classes on
my own free will and volition. Thus, I acknowledge that HOLY CHILD College of Davao
will not be held responsible for any misconduct of my child in violation of the standards
and procedures set by the institution and beyond school premises. Furthermore, I hold
HOLY CHILD College of Davao, entirely free from any liability or responsibility in the event
that my child contract COVID-19 during the conduct of limited face-to-face classes in SY
2022-2023.
