Under the recent guidelines issued by Ministry of Health and Family Welfare, Citizens above 45 years
of age are eligible to receive the vaccination in phase III.

The Union Health Ministry’s initial operational guidelines charts out mandatory pre-registration to
receive the jab on co-win app, Aarogya Setu app or on www.cowin.gov.in/home. Mediclinic promise

o Mobile with registered mobile number for SMS validation
o Copy of Aadhar card or any other Government ID proof
o Employee ID card
o Carry your co-morbidity prescription
 Please follow observation area guidelines for 30 minutes
 Report our team in case of any physical discomfort
