Embraer Air Safety Reports Summary to Operators

Dear Operator,

The Embraer Air Safety Reports Summary to Operators is a compilation of selected reported events
involving Embraer aircraft. It is issued regularly with the sole objective of enhancing safety
awareness at Embraer aircraft operators. Each report contains the relevant technical and
operational information of the event, whereas the details that might identify the concerned operator
are intentionally suppressed.

Embraer encourages the dissemination of this Summary to key individuals within your organization
who you feel can use it for its intended objective. Disclosure beyond this limit, either of the whole
document or parts of it, is not allowed unless duly authorized in writing by Embraer.

Embraer emphasizes the importance of communicating Air Safety Reports or Service Difficulty
Reports to the manufacturer. Once received at Embraer, these reports are processed to monitor
trends and evaluate the need for actions. If your organization does not regularly send this kind of
report to Embraer, we encourage you to consider the adoption of this practice, as instructed by
SNL145-00-0009.

Subj: Report of aircraft pulling to the right during taxi and landing
Desc: Flight crew reported that the Acft pulled to the right during taxi and landing. Maintenance personnel removed and replaced the
feedback potentiometer IAW AMM 32-50-07. Subsequent operational check were satisfactory.

Desc: Flight crew reported that the acft pulled to the right (randomly) during taxi. Maintenance personnel removed and replaced the
steering tiller. Subsequent operational check AMM 32-50-01 were satisfactory.

Subj: Report of cabin altitude Hi
Desc: The flight crew reported cabin altitude rising 2800ft per minute while cruising FL170.
Maintenance personnel found a broken sleeve connecting to #2 Air Cycle Machine. The broken sleeve was removed and
replaced

Subj: Report of emergency declared during approach due to abnormal landing gear indication
Desc: The flight crew reported an abnormal landing gear indication (down selection) during approach. Then they declared
emergency and requested a fly-past inspection for ATC. The ATC confirmed that the LDG appeared to be fully extended. The
flight crew landed without incidents.

Maintenance personnel removed and replaced the Landing Gear Electronic Unit (LGEU). Subsequent operational checks were
performed satisfactory.

Subj: Report of smoke on board
Desc: The flight crew reported smoke in the cabin. They noticed that the origin was from rear to the middle of the aircraft, with a
strong burning smell.

Subj: Report of uncommanded engine 2 IFSD during descent
Desc: The flight crew reported an abnormal noise on engine 2 during descent, followed by an uncommanded engine 2 IFSD. An
uneventful single engine landing was performed.

Maintenance personnel found metal pieces in the compressor inlet during the engine visual inspection.

Subj: Report of precautionary engine 2 IFSD due to abnormal vibration and engine oil low level indication during approach
Desc: The flight crew reported an engine 2 abnormal vibration and low oil level indication during approach. They followed the QRH
procedure and elected to precautionarily shut the engine 2 down.

Maintenance personnel found the engine locked (could not be manually rotated) and identified the oil leakage in the bearing
#0. The engine 2 was removed and replaced.

Maintenance Personnel removed and replaced #1 engine carbon seal and fan drive shaft.

Desc: The flight crew reported fumes and small amount of smoke in the cockpit in row 1 to 5. They also felt thuming under floor
around row 3 to 4.

Desc: Flight crew reported that the Acft pulled to the right during taxi with tiller depressed. The maintenance personnel did the
adjustment of NLG steering potentiometer per AMM 32-50-00. Subsequent operational checks were satisfactory.
