A. HISTORY

Gestalt Psychological Services Company have been started serving

different companies/agencies both private and government and individuals in the

entire CARAGA Region and even outside CARAGA in neuropsychological and

psychological exam, psychological evaluation for Annulment Adoption,

VAWC, , Kids with Special Needs (i.e Autism/ADHD & Learning Disabilities),

HR Seminars & training and counseling/ psychotherapy since 2010 up to the

present.

II. CLINICAL SETTING EXPOSURE

A. NATURE OF THE JOB OF THE TRAINEE

The interns’ duties include providing support to the employees, doing

clinical interviews and tests administrations for the client, and writing a

psychological report.

The intern is assigned to do clinical interviews with the clients and test

administrations as well as write a psychological report. Each client will be

interviewed and take tests according to what they need, the intern will then score

and interpret the tests taken by the client and after the compilation of the

interview and tests results was done, the intern will write the report to present to

the client he/her results. The intern and the client will then discuss the results of

the interview and tests during the presentation and ask follow-up questions. After

adding the necessary information that has been gathered from the client, the intern

will present the report to the supervisor of the internship. The supervisor will give

advice on how to further improve the report and the intern will revise the report

after and pass it to the supervisor.

As a mental health status check-up for the client’s job-seeking, she was referred

by a colleague as she is currently looking for a job. As such, the client, a 33-year-old

mom seeking a job, was interviewed and would like to know how she would handle her

stress if she experienced it. As she is a mom, she would like to have a job that has

flexible hours so she can still have time to spend with her children. She would also like to

be aware of her strengths and weaknesses in addition to what she already knows.

The client took these tests; Intelligence Test to measure the client’s intelligence,

Myers-Briggs Type Indicator to assess the client’s personality, The Quick Emotional

Intelligence Self-Assessment to assess the client’s emotional intelligence, Basic

Personality Inventory or Minnesota Multiphasic Personality Inventory to assess the

client’s tendencies for maladjustments/disorders, and for projective tests, the client has

done the Draw-A-person test, House-Tree-Person and Sachs Sentence Completion Test to

uncover feelings, desires, and conflicts that are hidden from conscious awareness. By

interpreting responses to ambiguous cues, psychoanalysts hope to help the client in her

employment. Hence, the evaluation and assessment results will be helpful for possible

interventions that can address the client’s concerns if there are any.

III. BACKGROUND INFORMATION

Mrs. C.M is the first child out of five and graduated with a course in Computer

Science at the University of Manila. She married her husband in the year 2017 and is a

full-time mother of two girls, the oldest being 10 years old and the youngest being 4

years old. She mentioned that she has asthma, which is one of the reasons she left her

previous job, and it has been a year since she left. She became ill for more than a week

and decided to rest and leave her job; additionally, the client stated that she suffers from

asthma and that during the pandemic outbreak, she used to overthink, get nervous, and

experience mild panic. She mistook her mild panic back then as the cause of her asthma

when it was actually the mild stress she was experiencing. She even mentioned that when

she had a mild panic and could not breathe, they even went to the hospital as she was

nervous and thought she might have COVID, but the doctor prescribed her to take

Gaviscon (a medicine used to treat acid reflux and indigestion) and that she felt better

afterward.  

She said that as things are slightly going back to normal, she would like to have

work again, as having no work means she has free time and no income. She thought of

finding a flexible full-time job to have income that she could use to pay bills, need in the

house, for her children to use, etc.

IV. BEHAVIOR OBSERVATIONS

Mrs. C.M prepared and made herself presentable. She was a bit nervous but after a

while, she relaxed. Mrs. C.M was ready for the interview when we arrived at her house.

She was dressed casually in butterfly printed shirts, had her hair tied in a ponytail, and

appeared to have good hygiene. She was well-dressed and appeared to be in good health.

She was oriented to person, time, and purpose, and had good eye contact throughout. She

was interviewed for approximately 15 minutes, or less than a half-hour. Her primary

language is Tagalog and she spoke in a normal tone, volume, and rate of speech. Mrs.

C.M appeared to have an excellent attention span, and a warm, pleasant and friendly

mood, which made it easy to establish rapport. She listened carefully and calmly

answered the questions that were asked to her, and she elaborated when necessary.

Therefore, the evaluation appears to be a reliable indicator that the Information she

shared and presented appeared to be generally accurate.

A. General Appearance: A 33-year-old female Filipino who appears to be the

stated age, looking neat and clean, chubby, the dark circles under her eyes are

visible and wearing a butterfly printed shirt.

B. Behavior: The client gives accurate responses to the questions, and is active and

cooperative during the interview.

C. Emotions and Affect: Pleasant mood and affect during the interview that is

appropriate to the context.

D. Stream of talk - Talks slowly at times and quickly at others, depending on the

Interview

The client was asked several questions during the interview. The first question

was asked after talking to the client, What characteristics do you see in yourself and

confident that you have these characteristics? She answered that she was a flexible

person and a quick learner. She was then asked what are her weaknesses that she

thinks she can develop. She answered that she was an overthinker of negative things but

she hopes to not overthink negative things and next time, she wants to think in a positive

way. Then she was asked what are her plans or strategies to enhance the weaknesses

she just mentioned, she answered that she tends to not think, to not think about things

that are not happening right now and not worry about the future. During this part, she

laughed and was comfortable talking to us. She listened attentively and answered after

thinking about our questions.

She was then asked what are the problems she encountered during her

searching for a job, which she answered that it was finding work as she is a working

mom and she wants a work where there is no work in weekends to have time with her

kids and has not a strict working environment but she said that there is no such work and

also mentioned that can she be picky because she was the one who needs the work and

that’s what she finds hard. When she was asked what are the things that you want to

know before you apply to the job, she pondered for a while and answered that she

wanted to know what kind of work she was applying to, if she can do the responsibilities

or not as she does not want to disappoint the people who will hire her. After that, she was

asked what she wanted to know about herself before applying for a job. She

answered that she wanted to know and challenge herself where her capabilities and

limitations lie as she is already old, she wanted to explore her capabilities. Next she was

asked what her plans are in finding a job right now which she paused and thought for

a while, she then answered with a carefree laugh, that she just wanted to find work to

support the needs of her children and to pay her bills.

Based on the results of her Intelligence-Quotient Test, she got a score of 11 out of

25 with an IQ conversion of 94. Implying that she is Intellectually average, signifying

her proficiency to complete job-related tasks that required both cognitive and physical

abilities recognizing patterns in shapes and figures.

In the result of Myers-Briggs Type Indicator, her scores were interpreted and fell

under ESFJ (Extraverted Sensing Feeling Judging). Based on the test result, she has a

personality type of “The Caregiver” or “The Consul”. The Consul has various

characteristics including strengths and weaknesses which can be observed in the client.

According to it, a Consul (ESFJ) has strong practical skills, a strong sense of duty is very

loyal, sensitive, and warm, and is good at connecting with others which could be true

because the client stated during the interview that she is flexible and able to learn quickly

when someone shows her how to do something or assigns her a task. Also, this

personality type is one of the few that is both practical and caring. Consuls are well-

known for their social and administrative abilities, and they excel at creating and

maintaining a safe, stable, and friendly environment for themselves and their loved ones.

Consuls’ commitment is invaluable in many areas, including their own personal

development.

When it comes to the workplace, consuls have distinct tendencies that manifest

themselves regardless of their position. People with the Consul personality type thrive on

social order and harmony, and they use their warmth and social intelligence to ensure that

everyone is aware of their responsibilities and capable of getting the job done. Because

Consuls’ characteristics are so strongly expressed, leading with practical sense and social

vigor, the careers they find most fulfilling usually revolve around making the best use of

their qualities. Consuls are well-organized individuals who enjoy bringing order and

structure to their workplaces. Consuls often perform best in environments with clear,

predictable hierarchies and tasks. Monotony and routine work do not present a problem

for the Consul personality type, as they are content to do what is required. In addition,

ESFJs possess a wide range of abilities that can benefit those around them significantly.

Their enthusiasm and energy can have a positive influence on those around them. The

strengths of ESFJs make others feel welcomed and motivated, which can lead to more

productive and happy lives. Making sure others feel cared for, paying careful attention to

the details of an event, considering the impact a change will have on the group,

developing and seeing the potential of team members, and providing a warm, stabilizing

presence for others are all traits associated with the ESFJ personality type. When ESFJs

engage in tasks in which they excel, they are happier and more productive. They’ll be

able to actively engage with their work while also utilizing their strengths, which may

help them stay with a position in the long term.

Lastly, in accordance with MBTI, the weaknesses of a person with an ESFJ

personality type can negatively impact them. If ESFJ’s do not learn to manage or

overcome potential areas of weakness, their strengths will be overshadowed and their

positive impact will be diminished. The weaknesses associated with the ESFJ personality

type include a strong desire to avoid conflict, being easily thrown off balance by

emotionally charged situations, struggling to give negative feedback, and making

unpopular decisions. Which could be true because she mentioned during the interview

that she used to overthink, and she usually overthinks negative things. Thus, When ESFJs

make the effort to identify and overcome their blind spots, they can make strides toward

self-improvement. Small, incremental changes can lead to significant growth and long-

term development.

The client’s strongest quadrant in the results of her emotional intelligence self-

assessment is Emotional Management, with a score of 36 falls under enhanced skills,

indicating that she has self-control, transparency, initiative and adaptability. Ther is

supported by her results, in which she ranked 4 (always) the statements, “I control urges

to overindulge in things that could be damage my well-being” and “I can restrain myself

when I feel anger towards someone”, also during the interview, she also mentioned that

when she overthinks negatively, she tends to focus on what is more important in her life

and stops herself from thinking negatively. It could imply that she has self-control. With

a score of 35, her second strongest quadrant is Social Emotional Awareness, which falls

under enhanced skills. In which she is aware of other people’s problems, needs, and

feelings and does her best to help them. Indicating that she is competent in terms of

situational awareness, empathy, and service ethics. Next quadrant was her Relationship

Management with a score 32, falling under effective functioning. It implies that her

potential capabilities are communication, conflict management, interpersonal

effectiveness, and building bonds. Lastly, her lowest-ranked quadrant is Emotional

she affirmed that she often worries about her health, sometimes she feels bothered by the

pains in her chest or heart, neck, and head, as well as feel dizzy or have headaches and

migraines. She has a raw score of 25 and a T score of 59 in Depression (D) which

indicate that she feels that sometimes she is no good as other people, but she still feels

life is worthwhile. She has a raw score of 25 and a T score of 77 in Masculinity-

Feminity (Mf) which indicates she rejects traditional feminine roles. As she is a mom,

she believes that she still needs to find work for her family unlike the traditional mom

where they stay in their home to take care of her kids and do chores. She has a raw score

of 32 and a T score of 59 in Psychasthenia (Pt), which specifies that she is more nervous

than others and usually feels anxious about something or someone almost all the time

which Mrs. CM affirmed in the interview that during the start of the pandemic she

thought that she experienced a panic attack due to her anxiety. She is also easily

awakened by the noise, but she has a good appetite. She has a raw score of 31 and a T

score of 59 in Schizophrenia (Sc), which indicates she has limited interest. Lastly, she

has a raw score of 44 and a T score of 67 in Social Introversion (Si) which specifies that

she believes that most people make friends because friends are likely to be beneficial to

them, and she is introverted.

figure, and it is the representation of herself. She describes her drawing as "I’m happy

and contented right now." The placement of the first figure that she draws on the left side

of the paper indicates her past. She draws her head in the center, but it is

disproportionate, suggesting that she might have difficulty in one area and brain damage,

severe headaches, or other head sensitivity. Her drawing has a neck presence,

representing the connection between the head and the body; however it is short which

detail that shows her severe anxiety. She said that she lives in the house and that it is a

relatively normal, which means she is less conscious, and there are a few fruits as

dependent children and pregnant women draw fruits and there is a presence of a trunk,

indicating the person's ego strength. The branches show intense hostility, and the visible

roots below the tree emphasize ungratified needs and she has drawn crude leaves which

indicates less intelligence. She said that the tree is an alive Narra tree and it is 1,000 years

old, she said it was currently summer in her drawing and she is the one watering the tree.

Lastly, she draws a person who represents a self-portrait. It is tiny, and unlike her

other drawings, it implies that the person feels small, and she responds to the demands of

the environment with feelings of inferiority. Also, she draws a small head; it may mark

inferiority feelings or denial of the site of guilt feeling. Since she was the only one in the

picture, it denotes a lack of family support or the absence of a family support system

altogether. The drawing is placed in the lower right portion of the page, which

symbolizes over-anxiety about involvement. Her rendition of herself appears very

trying to hide or withdraw from something within the family and as she is sitting,

frequently indicative of low energy level, lack of drive, or of emotional exhaustion as

well as short arms which suggest an absence of striving. She said that the person in the

drawing is her, she is currently 33 years old and is feeling happy as she watches

television.

VII. INTERVENTIONS AND RESULTS

She stated in the interview that she has a tendency to overthink, and that she

usually overthinks negative things, which is a manifestation of anxiety, mental and

emotional stress. According to Cohen (2019), Stress can come in different forms such as

physical, mental, and emotional where experiencing it is normal as it is a natural part of

life. Stressors that appear occasionally may be harmless in our well-being however, when

it becomes persistent, it could lead to a long-term health problem. Mrs. C.M's life is

currently being impacted by stress, which is having a negative impact on her functioning.

She has a tendency to overthink things, and she is worried that her husband's income is

not enough for them, so she thought of finding a full-time job for weekly income, and she

wrote on the Sachs sentence test that she is afraid of the dark. Her lack of sleep is also

visible in the form of dark circles around her eyes, as we observed, indicating that she is

having difficulty sleeping. Concerning Cohen's list of effective ways to cope with stress,

which could be used by the client in order to help herself, one of which is

compartmentalizing thoughts. It is done by separating her thoughts into different areas

and leaving them where they belong to avoid overthinking and worrying. The second one

is called focus. It is simply focusing on one problem at a time, which helps the person to

take control over their thoughts and avoid the overflowing of stressors. After finishing it,

take your time to rest and relax before jumping to another one. Simple activities such as

taking a walk, drinking tea, eating, or meditating can help her clear her mind while also

allowing her body to rest.

VIII. SUMMARY

As a mental health status check-up for the client seeking a job and referred by a

colleague has undergone an interview and tests. Mrs. C.M is the first child out of five and

graduated with a course in Computer Science at the University of Manila. She married

her husband in the year 2017 and is a mother of two girls, the oldest being 10 years old

and the youngest being 4 years old. She is currently looking for a job in order to pay their

bills as their children grow older and goods become more expensive. As for her health

she mentioned that she has asthma.

Based on the psychological test results and the information gathered during the

interview with Mrs. C.M as well as her results in EQ test, her emotional ability was fairly

high, as her verbal interpretation falls under Effective Functioning and Enhanced skills

based on her performance in the test, where she received a significantly high score which

corresponds to her result in MBTI. As per the results on her Intelligence Quotient (IQ)

test, she has no any intellectual disability and there were also no problems related to it

that has been reported. In connection to that, as she has a high score on the emotional

quotient ability test, her result in MBTI’s strength which determines her personality, fell

under ESFJ (Extraverted Sensing Feeling Judging), wherein having a personality type

like this is someone who is "The Consul'' or "The Caregiver," can make others feel

welcomed and motivated, which can lead to more productive and happy lives. Making

others feel cared for, paying close attention to details of an event, considering the impact

of a change on the group, and maintaining a stable presence can all serve as evidence that

she has emotional control over and a personality that is sensitive to people and

her surroundings.

In the result of her Basic Personality Inventory/MMPI, it is shown that she has a

tendency to have the disorders mentioned. Mrs. CM got a score of 77 in Masculinity-

Femininity (Mf), which is the highest among the domains that arise on her MMPI, where

the symptoms are to reject traditional feminine or masculine roles, which corresponds to

what Mrs. CM's situation is where she is trying to find a job to help support her family

and have an income. We concluded that she got the highest score in this domain as, being

a mother of two, she rejected the Domesticated role of a mother that should be at home

taking care of her daughters and her husband.

Next, she has a score of 69 in Hypochondriasis (Hs), where one of the symptoms

is that she may develop somatic symptoms in times of stress. She did mention having

headaches or migraines at times, feeling chest pains, and acid reflux. The client’s trouble

breathing might also be part of hypochondriasis symptoms due to stress as according to

her results in DAP, she is reserved and might have been suffering from within and

according to her MBTI, she might have repressed her issues to herself and would not

express it but the body has a way of expressing the client’s emotional tension. As we can

also relate to the score of her psychasthenia (Pt), she got a score of 59, although it was

not entered into the 60 and above basis to be interpreted. But because of having a related

connection to (Hs), we would like to include it as well. So much for that, the

interpretation of psychasthenia (Pt) having these symptoms such as being anxious, tense,

and uncomfortable; and, as previously stated, she experiences mild panic, particularly

during pandemic outbreaks. She was having trouble breathing. Her overthinking of

negative things also further added to her anxiety, and she immediately visited the hospital

to check but was sent home and told to take Gaviscon by the doctor. 

Lastly, she got a score of 67 in Social Introversion (Si), indicating that she is

introverted, depressed, guilty, slow personal tempo; lacks self-confidence; lacks interests;

submissive, compliant, emotionally over-controlled. As we can connect to the result of

her MBTI, wherein the ESFJ personality does not learn to manage or overcome potential

areas of weakness, their strengths will be overshadowed and their positive impact will be

diminished. We could say that if a sudden situation happens which she cannot handle, the

client's social introversion will emerge. For instance, due to the pandemic, she has grown

used to being inside the house.

However, we found a conflict as we mentioned that she has a personality type of

ESFJ, which is "extroverted," and her results from MMPI showed she got a high score in

"social introversion" (Si), which makes a contradiction. Hence, we concluded and were

very much sure that the client was extraverted as during the interview she was shown in a

warm, pleasant, and friendly mood, which made it easy to establish rapport. Also, in her

Draw-a-person test, the hands of her drawing were wide open and placed outward. With

that, it could validate her extraversion.

The results of her projective tests. In Sachs Sentence Completion Test, most of

Mrs. C.M’s degrees of disturbance in every subject have no disruption. There are only

five mildly disturbed interpretations, specifically in the client’s area of fear, guilt

feelings, goals, and attitude toward the past and future. The mildly disturbing

interpretation means the client has no issues handling problems without the aid of a

mental health professional but there is still emotional conflict in the area and her results

in the Draw a Person Test, were quite low and resulted in a score that was below average.

towards other people. It was observable during the interview as well, as she was someone

whom you can talk to comfortably, polite, and attentive. As corresponds to the results of

her MBTI, she is very much loyal to her friends and family as well based on the

experiences she shared during the interview; she was a dependable one where she would

be there if one of them needed help. That personality of hers manifests because she is the

first child or the breadwinner in her family.

Despite having many positive qualities, she admitted in the interview that she has

a tendency to overthink, and she usually overthinks negative things. However, she was

able to overcome this by stating that when she overthinks negatively, she tends to focus

on what is more important in her life and stops herself from thinking negatively which is

a great asset that many employers want. she was mentioned that she has feelings of

inferiority, fear of responsibility, indecisiveness, and guilt feelings, these might be related

to her past concerns, also she is still unsure of her goals and future but she is reality-

oriented and has a good relationship with the environment, there can be seen from her

previous job where she is a technical support that assists clients. She has more experience

in IT and considers herself as having expertise in this field, and she is happy to help and

assist her customers with their concerns about the software. She would like to work for a

company that would maximize her expertise and allows her to widen her experience.

IX. RECOMMENDATIONS

It is highly recommended for her and her family to undergo family counseling to

address her concerns and also to discuss her plan to look for a job. In this way, the client

can further understand her strengths and limitations as well as her family connections,

which may help to improve her well-being. It is also suggested for the client to take

frequent breaks and pause from doing strenuous activities to avoid exhaustion. The client

must also have a regular checkup to evaluate her physical health since she mentioned she

is prone to respiratory illnesses and has asthma.

Nevertheless, if the experience of Mrs. C.M. continues, becomes out of control,

and has already had a significant impact on her life, it is recommended for her to seek

help from mental health professionals such as counselors, psychologists, or

psychotherapists. Those professionals can be someone she can rely on and openly talk

about her stress. She could also learn to identify the triggers of her stressors, and through

that, a professional could create the right plan and interventions in order to help her

overcome them, as well as counseling to balance her work, health, and family.

It is also recommended for the client to actively search for a flexible job wherein

her experience and expertise will be maximized. We recommend that she work as a

virtual assistant, specifically as a technical support assistant. After further analyzing her

interests, strengths, and other aspects, it is perfectly aligned with her field of expertise, as

she is a computer science graduate with integrated software and application knowledge.

In connection with the reason for the referral, she mentioned that she is also looking for a

job that has flexible hours because she is also a mother of two. We concluded that this

type of job is very much suitable for her as the company that is related to virtually

assisting clients is ready to help clients 24 hours a day, which means nighttime hours are

definitely possible. She could work on a ship at night and still keep an eye on her

daughter during the day. And also, work related to virtually assisting clients offers work

from home, which is much to her favor. Lastly, we assume that the psychological test we

administered to her was insufficient, and we suggest that she take additional tests such as

an aptitude test, occupational interest tests, career personality tests, and subject

I. IDENTIFYING DATA AND REASON FOR REFERRAL

Mr. Joshua was referred to by a colleague as he was experiencing relapse of Major

Depressive Disorder. The client is a 22-year-old male who was clinically diagnosed last

2019 and took medicine and therapy in a clinic in Quezon City. Hence the client was

subjected to 3 objective tests namely (1) IQ Test, which is a 25-item culture-free visual

test; (2) The Quick Emotional Intelligence Self-Assessment Test to test his emotional

strengths and weaknesses; (3) The Big Five Personality Test to identify the strengths and

weaknesses among 5 facets of personality namely Openness, Conscientiousness,

Extraversion, Agreeableness, and Neuroticism. The client also undergone 3 projective

tests namely: (1) House-Tree-Person test to identify on how the client implacably

perceive himself and his environment; (2) Draw-A-Person test to check the levels of his

anxiety and to validate his IQ and lastly, (3) The Sachs Sentence Completion Test to

identify his views on his own, his family and his surrounding environment. The results of

these tests may be a referral guide for further analysis of Mr. Joshua’s mental status.

III. BACKGROUND INFORMATION

The client is a 22-year-old College student studying Bachelor of Arts in Mass

Communication in the University of Diliman. He was the youngest son, and has only 1

brother. He lives with his mother, his brother, two aunts and one cousin. He is actively

participating in youth organizations in their school particularly in relation to media in

which he is one of the leaders. He is physically healthy but has been experiencing a

relapse of a Major Depressive Disorder and hasn’t seen his therapist for 2 years. He also

paused 1 year in 2021 in his school due to his mental condition. Recently, he is still

pursuing his college degree as a 3rd year and aspires to become a writer in alternative

media. 

IV. BEHAVIOR OBSERVATIONS

The client wore a casual black printed shirt and appeared to have good hygiene. 

He was well-dressed but it was noticeable that cloth wrapped around his wrist, despite

appearing to be in good health. He was asked about it during establishing rapport but he

refused to explain what happened. He was interviewed for approximately 17 minutes. His

primary language is Tagalog and he spoke in a normal tone, volume, and rate of speech. 

During the course of the interview, he appeared to have an excellent attention span, a

warm, pleasant, and friendly mood, which made it easy to establish rapport. He listened

carefully and assertively answered the questions that were asked to him, and he elaborate

his answers when necessary.

B. Information assessment Techniques

Interview

The client was asked several questions after the discussion of his results in the

objective and projective tests. The first question pertained to how is his mental health

these days and he promptly responded he doesn’t know but he knows he acknowledges

he is not okay. He was asked to rate on a scale of 1-5, 1 being low and 5 being mentally

healthy, he answered 2.5 and he feels mentally exhausted because of the demands in

his academic responsibilities. He was also asked an implied question what does he think

triggers the breakdown of mental health of youth these days and he answered 3 main

factors, family problems, academic responsibilities and social relationships. And then he

was asked if this also was implied to him and he responded somehow. The client during

the interview felt comfortable sharing his thoughts and composed himself without

getting too emotional given the interview was administered in a public area. 

He was then asked how is he coping up and is it working and he said that

sometimes, he just needed rest for a week or two and it cost him his leave of absences

in school. When he was asked about how much rest does he think he needs, he

answered minimum of 6 months up to 3 years to completely rest and gain energy again.

He was looking to rest for a while after his graduation before entering an employment.

He was also asked if there are people whom he can communicate with his feelings, he

mentioned that he has friends whom he can share his thoughts with but he has

mentioned that darkness inside his head is too loud sometimes. He acknowledges the

people around him but sometimes he feels that his own thoughts were “eating” him and

leading to suicidal tendency. He was asked the last question what are the things he

need right now and he responded that he might probably need mental medication and

therapy again but he is still considering time and resources.

emotions and feelings; he can manage and control them and he can also maintain good

relationships with other people.

During the interview dated May 2, 2022, he was asked follow up questions with

regards to this Eq test.  The first question was about how he demonstrates his strongest

domain which is Social Emotional Awareness. He then stated that whenever he is in the

crowd, he tends to feel the presence of other people. He feels their aura and wonders

how that person feels, or if he can jam with the person he is with. The next question

tackles his weakest domain which is emotional management and how it affects his life.

On the quantitative part, it seemed that he is under the category of functioning but

during the interview, he mentioned that his loss of control to the things he feels and to

the thoughts coming in, whenever he feels overwhelmed by his responsibilities, he

intentionally misses his classes and it somehow comforts him. But the stress that he

feels comes back when there is a lot of work to do. He then still accomplishes it before

the deadline. The last question focuses on improving his Emotional Management and he

said, he feels derailed, he just sometimes needed rest and acknowledgement.

Whenever he encounters conflict, he will first try to acknowledge where he went wrong,

rest for a while whenever overwhelmed and find ways on how to get back on track.

The score of the client were as follows: for Extraversion, he scored 18 with 

mean of 2.25 and can be interpreted as low; for Agreeableness, he scored 35 with a

mean of  3.88 with interpretation of high; for Conscientiousness, he scored 26 and had

a mean of 2.88 which is moderate; in Neuroticism he scored 35 with a mean of 4.37

which is high and lastly for the Openness, he scored 35 with a mean of 3.5, interpreted

as moderate. It is evident for the client that he has high levels of anxiety and since he

scored low on extraversion meaning he doesn’t have much energy to do things. This is

evident through a follow up interview conducted last May 2, 2022 that he is

experiencing relapse of his Major Depressive Disorder and is highly anxious these days.

It greatly affects his academics and is not motivated to do school work. His score on

Most of the client’s degrees of disturbance in every subject have no disruption or

disturbance. There are only 6 mildly disturbed interpretations, specifically in the client’s

area of guilt feelings, attitudes toward the mother, family unit, own abilities, the past

and future. There is one area of the client’s fears that seems needed to be worked on

with a professional or needed to be interviewed more for further investigation since the

interpretation is severely disturbed. The mildly disturbed interpretation means the

client has no issues handling problems without the aid of mental health professionals

but still emotional conflict in the area.

         For the detailed discussion of every mildly disturbed and severely disturbed area.

Going first with the mildly disturbed, based on the score, the client sees mother’s fault

but accepts and tolerates differences with regards to the attitude to his mother. The

client’s aware that the family does not recognize him as a mature person but has no

difficulty in relating with them for the attitude towards the family unit. The client has

regret over the past and seems mildly disturbed by his failure to control his trouble in

the guilt feeling area. He feels he has a specific ability but tends to fear difficulty for his

attitude towards own abilities area. Regarding his past, he is in the boundary of feeling

rejected/isolated and feels well adjusted, having another interview is helpful to

determine the weight of his answers. He is unsure of himself but tries to be optimistic

for his attitude towards his future. The client’s fears are the severely disturbed area, he

is disturbed by the apparent fear of loving, possibility to control his feelings. Further

evidence can be gathered in his next interview session to review the findings for more

accuracy.

The client drew a man first. He described this as a 25-year-old middle aged man

who struggles to identify himself with the surroundings. Upon validating each body part

and segment in the drawing, he obtained a score of 29 and it is equivalent to an IQ of 94

with an approximate percentage of 16.12.

The Person drawn was in the center of the paper, no significant disturbances

found. The shape of the head is drawn proportionately and no sign of malfunction with

the brain. The neck represents clear connection between head and body, no over or

underemphasis seen. It is also noticeable that he completely omitted the face which

signifies avoidance of social problems. The arms and hands are detailed and exposed,

representing ego development and social adaptation. Extended arms represent a good

relationship with the environment or spontaneity. And lastly the fingers are well

defined. Usually represents aggression. Fingers on the right side from the perspective of

the drawing seemed longer than the left. He might have torn aggression. Nails are not

present and it represents being reserved but all his fingers are open.

The initial drawing of Mr. Joshua is a massive tree with no trunk visible, six (6)

branches, and leaves, but it is not fully detailed. He also drew a house with a chimney,

as well as a single door and a single locked window. His home also has a modest gate

and a fence. However, the person he drew is not highly defined. The person he drew is

outside the house walking under the tree.

Mr. Joshua drew a simple tree portrait, which is a two-dimensional tree without

a visible trunk but an excessively huge number of leaves. Trunk represents basic power,

or a person's ego; he drew a tree without a visible trunk. However, the leaves he drew,

on the other hand, are rough or crudely drawn, implying that he is less intelligent; leaves

represent fulfillment and satisfaction. Mr. Joshua also drew a home with an earlike roof,

which represents auditory senses, and a wall, which represents ego, and a light pencil

line, which suggests he does not want to become involved. The house also has a simple

chimney, but no smoke coming out of it, indicating that the house is cool and that his

father is missing. A single door is also included in the house he drew; the door

represents direct contact. The house also has one window, which refers to the various

ways in which people interact with their surroundings. The window had no panes and

was blank, indicating opposing tendencies and drew in light in a strokes manner, which

it denotes apathy and an empty sensation within. He also placed a fence or a gate in

front of the house, which symbolizes guardedness and the ability to protect oneself

from others. Finally, He drew the individual which he did not include much detail in his

drawing. The person is just walking under the tree, and when it comes to movement, He

drew a mechanical kind of figure which means completely lacking in kinesthetic

implications, the analyst should be alert for other signs of depersonalization and

psychosis.

VII. INTERVENTIONS AND RESULT

The client mentioned that his mental health was 1, which is the lowest when we

asked him to scale his mental health, 1 being the lowest, 5 being the highest. The client

has the tendency to worsen each day if there is something triggering their emotional and

mental well-being. This can also be patterned to the client's diagnosis of depression.

Therefore, The psychotherapy that is good for the client is CBT (Cognitive Behavioral

Therapy). It would be very beneficial to the client because it will assist her in identifying

unhealthy, negative beliefs and behaviors and replacing them with healthy and positive

ones.

VIII. SUMMARY

As a mental health status check-up, the client has undergone an interview and

tests. The client is a 22-year-old College student studying Bachelor of Arts in Mass

Communication in the University of Diliman. He was the youngest son, and has only 1

brother. He lives with his mother, his brother, two aunts and one cousin. He is actively

participating in youth organizations in their school particularly in relation to media in

which he is one of the leaders. He is physically healthy but has been experiencing a

relapse of a Major Depressive Disorder and hasn’t seen his therapist for 2 years. He also

paused 1 year in 2021 in his school due to his mental condition. Recently, he is still

pursuing his college degree as a 3rd year student and aspires to become a writer in

alternative media.

The results of the psychological tests administered to the client seemed to be

normal and minimal disturbances found. On the objective tests, he has a very superior

IQ, and Effective functioning of Emotional Quotient. The result of his personality shows

that he has a high neuroticism and low extraversion that significantly shows on his

behavioral interviews. On his IQ, he has obtained a very superior score but when DAP

was administered it was on Average level. This still shows within normal range of his

Intelligence quotient. 

The client has a great attention span, warm, pleasant and friendly which is

comfortable to communicate to. He is very active in school, smart and has a clear goal,

he also has a high agreeableness which corresponds to his projective tests such as he

has a good relationship with the environment but he avoids social problems and keeps

his problems to himself as he is also reserved which corresponds that he has low

extraversion and may show aggression as according to his HTP results, he is guarded, a

tendency to protect himself, does not like to be involved and may have signs of

depersonalization and psychosis as the client has a high neuroticism and low emotional

management. In his EQ test, his social emotional awareness was the highest, which

corresponds to the fact that he is sensitive to the feelings of other people and in his HTP

shows that he has a good social adaptation, however this might indicate that he will be

more influenced by his environment  and other people which may be one of the causes

of the client’s relapsed. 

IX. RECOMMENDATIONS

● As the client has stopped meeting his therapist, It is recommended to continue

as well as to continue taking his medicine for his treatment as to prevent any

relapse in the future. It is advised for the client to exercise and to rewind himself,

give himself some time to rest. 

● Journaling is also recommended in order to aid Mr. Joshua in processing his

thoughts and emotions. Writing down his specific ideas can be a way of sorting

out her train of feelings.

● As the client's score was interpreted as very superior with a score of 21 out of 25

in his Intelligence Quotient Test but lowered in the projective test (Draw A

Person), we recommended taking other mental ability tests (Raven's Progressive

Matrices) to test or support the validity he got on the test.
