As the Class Adviser of Grade 11 - Erubescens, I am pleased to inform you that your child CARLO
DOMINIC O. AGUILAR will receive With Honors award on the 26th Recognition Day at Naga City
Science High School on July 12, 2022, Tuesday, 10:00 in the morning at the Covered court. The
prescribed dress code for the said event is casual attire. All are expected to wear decent outfits. Hence,
males must be in polo shirts and slacks matched with black shoes and socks. Females, on the other
hand, are prohibited to wear skimpy shorts or skirts and revealing clothes.

In line with this, a rehearsal will be conducted on July 8, 2022, Friday, at 1:00 in the afternoon at the
NCSHS Covered court. Please accomplish the Parental Consent below to be submitted on the
aforementioned date. Attendance is a must.

Again, congratulations and thank you for your endless support and assistance in the pursuit of
academic excellence of your child.
