I, OKON-ETTE, AUGUSTINE EDEM Male, Christian, Adult,

Nigerian citizen and a Native of Aka Offot in Uyo Local Government
Area of Akwa Ibom state, now residing at No.40b Nepa Line, Uyo
do hereby declare an Oath and state as follows:

1. That I am the above-named person whose passport photograph

is attached.

2. That I was reliable informed by my parents and I verily

believed them that I was born on the 10th Day of November,
1951.

3. That my birth was not officially registered, hence this

affidavit.

4. That this Affidavit is now necessary for record and official

purposes and to enable any authority(ies) concerned to believe
the facts as deposed herein.

AND THAT I SWEAR to this Affidavit conscientiously, believing same to

be true and correct to the best of my knowledge and by virtue of the
provisions of the Oaths Acts of 2004.
