SOMATIC CELL GENE THERAPY - Therapeutic genes transferred into the somatic cells. E.g..
Introduction of genes into bone marrow cells, blood cells, skin cells etc. Will not be inherited later
generations. At present all researches are directed to correct genetic defects in somatic cells.
GERM LINE GENE THERAPY Therapeutic genes transferred into the germ cells. E.g.. Genes introduced
into eggs and sperms. It is heritable and passed on to later generations. For safety, ethical and
technical reasons, it is not being attempted at present.
Types of somatic cell gene therapy:
1.Ex vivo • cells are modified outside the body and then transplanted back in again • called
ex vivo because the cells are treated outside the body.
2. In vivo • genes are changed in cells when the cells are still in the body • called in vivo
because the gene is transferred to cells
(Adenosine deaminase (ADA) deficiency is an inherited disorder that damages the immune
system and causes severe combined immunodeficiency (SCID). People with SCID lack
virtually all immune protection from bacteria, viruses, and fungi.For the therapy, her white
blood cells were taken from her and inserted with the correct genes for making ADA and then
re injected into her.)
A. VIRAL VECTORS
These are Viruses that introduce their genetic material into the host cell as part of their replication
cycle. Here, the viral DNA is removed and the virus is used as a vehicle to deliver the therapeutic
DNA. The viruses used are altered to make them safe, although some risks still exist with gene
therapy.
What are the Types of Viral Vectors?
A number of viruses have been used for human gene therapy, including :
1. Retrovirus 2. Adenovirus 3. Adeno-associated virus 4. Herpes simplex virus
Gene Gun -Employs a high-pressure delivery system to shoot tissue with gold or tungsten particles
that are coated with DNA
Microinjection -Process of using a glass micropipette to insert microscopic substances into a single
living cell. Normally performed under a specialized optical microscope setup called a
micromanipulator.
b. LIPOFECTION
i. It is a technique used to inject genetic materials into a cell by means of liposomes.
ii. Liposomes are artificial phospholipid vesicles used to deliver a variety of molecules
including DNA into the cells.
DISADVATAGES
Long lasting therapy is not achieved by gene therapy; Due to rapid dividing of cells benefits of gene
therapy is short lived. Immune response to the transferred gene stimulates a potential risk to gene
therapy.
Disorders caused by defects in multiple genes cannot be treated effectively using gene therapy.
Viruses used as vectors for gene transfer may cause toxicity, immune responses, and inflammatory
reactions in the host.
ETHICAL ISSUES:
Who will have access to therapy?
Is it interfering with God’s plan?
Should people be allowed to use gene therapy to enhance basic human traits such as height,
intelligence etc.?
Is it alright to use the therapy in the prenatal stage of development in babies?
CONCLUSION
Theoretically, gene therapy is the permanent solution for genetic diseases.
But it has several complexities. At its current stage, it is not accessible to most people due to its
huge cost.
A breakthrough may come anytime and a day may come when almost every disease will have a
gene therapy
Gene therapy have the potential to revolutionize the practice of medicine.
