this Office directs all school heads to submit their list of established school
organizations on or before July 29, BO22.

2. The said report will be submitted to the Special Programs and Projects - Schools
Governance Operations Division.

Office of the Assistant Secretary for Youth Affairs and Special Concerns (OASYASC),
through the YFD, requests the submission of a consolidated list of all recognized
school co-curricular and extracurricular clubs and organizations to serve as
o AsvASC-YFD guide for the existing curricular activities and identify the roles and
responsibilities of each assigned unit/division overseeing it.

The Schools Division Office (SDO} must fill out the attached Annex A- Dtvtaton
