Directorate of Sports, UP on purely Contractual basis.

Dear Mr. JAGESAR SAINI,

We are pleased to inform you that, you are selected for the post of KHO-KHO INSTRUCTOR (ADHOC) on
the basis of your application received on Sewa Yojana Portal and the Selection/Trail Procedure
conducted on 12/03/21 as per guidelines of Directorate of Sports, Lucknow, UP. As per the selection list
and district allocation for the post received from Directorate of Sports, Lucknow, UP, you are selected for
the post of KHO-KHO INSTRUCTOR (ADHOC) at District Shrawasti.

You have to join on or before 25th April 2022 (joining date) for session 2022-23 according government
G.O. for sports department . Notwithstanding this, in the event of the project for which you are being
selected terminates before your contract end period, this contract shall be co-terminus with the project.

Your monthly consolidated salary would be Rs. 20000.00 (Rupees : Twenty Thousand Only) inclusive
of all statutory dues payable to you based on your attendance received and certified by Sports Authority
and your performance. You will be liable to carry out such works/duties as entrusted by Directorate of
Sports, Lucknow, UP and its supervisor from time to time.

Your contract may get terminated with immediate effect for misbehavior or found violating any
disciplinary norms.You will not claim employment or regularization of employment with Directorate of
Sports, Lucknow, UP or T&M Services Consulting Pvt Ltd .

You will work at locations allocated to you and participate in activities as directed by Directorate of
Sports, Lucknow, UP.

You will make your own arrangements for accommodation, food , local transport, etc. and you will not be
eligible for any additional claim or reimbursement in this regard.

In additions to the terms of appointment mentioned above, you are also governed by the terms and
conditions of the affidavit. The combined terms and conditions of this letter and affidavit constitute the
terms and conditions of appointment and you are required to read and understand both of them in
conjunction.

You must be report within 10 days after receving this latter, if not, your joining will be cancelled.
For the further procedure, kindly submit the scan copy of below documents on
bdo.lucknow@tnmhr.com & cc to hr.upsports@tnmhr.com before joining.

1) ID Proof (Aadhar card & Pan Card)

2) Bank Account Details (Passbook copy or Cancelled Cheque)
3) Medical Fitness Certificate from CMO
4) Non Objection Certificate (NOC) as per your affidavit additional term and condition point no. 3,4 .

5) Affidavit on Rs.50/- Stamp paper - Acceptance on Terms & Condition (Format Attached along with
this letter) and signed on all pages.
Note:- You have to submit affidavit copy at District Sports Office before joining. You have to send
original affidavit by speed post at T& M Services Consulting Pvt Ltd, D-32 , AMBIKA TOWER ,
VIBHUTI KHAND , LUCKNOW 226010.

If you fail to submit the above documents before joining, then your candidature for the post of KHO-KHO
INSTRUCTOR (ADHOC) will be cancelled and decision regarding this is solely with T&M Services
Consulting Pvt Ltd and Directorate of Sports, Lucknow, UP
