Application Form for Graduate Programs

(For Graduate Certificate, Graduate Diploma, Master's, and Doctorate Programs)

General Instructions:

This application shall not be acted upon unless the following documents are uploaded in this
Google Form on or before the deadline for submission of application documents. Furthermore,
this form requires applicants to upload their admission requirements, and once submitted, will no
longer allow edits. It is recommended to complete all admission requirements first before filling-
out this form.

Using a proper file naming convention for your admission requirements will help us evaluate your
application more efficiently, and we appreciate it more if you number them according to the
sequence below:

1. Correctly and completely accomplished application form for Graduate Programs

2. 500-words essay indicating your purpose for applying to the distance education programs and
your plans after completion of graduate study at the UP Open University.

3. Academic credentials, e.g. Transcript of Records (please refer to the OUR website for more
information: https://registrar.upou.edu.ph/admission );

4. Two (2) Letters of Recommendation from any of your employer/supervisor, former professor, or
former program adviser (download the recommendation forms at
https://registrar.upou.edu.ph/downloadable-forms ; References/Referees can submit the
recommendations as email attachment to admissions@upou.edu.ph )

5. Proof of payment of application fee (non-refundable) amounting to Php 500.00 for applicants
based in the Philippines or USD 100.00 for applicants based abroad (payment procedures:
https://registrar.upou.edu.ph/payment-of-fees/ );

6. Two pieces identical 2" x 2" photos and two identical 1" x 1" photos;

7. NSO or PSA Birth Certificate;

8. Government-issued IDs with photo;

9. Additional requirements for foreign applicants (non-Filipino)(please refer to the OUR website
for more information: https://registrar.upou.edu.ph/admission ).
10. Additional requirements for Diploma in International Health (DIH) and Doctor of
Communication (DCOMM) programs (please refer to the OUR website for more information:
https://registrar.upou.edu.ph/admission );

While online submission of application requirements is accepted for now, all admitted applicants
are required to submit the hard copy of admission requirements, especially the valid copy of TOR,
to the OUR before the start of the registration period in the 2nd Trimester/Semester AY 2020-
