PERINEAL CARE

DEFINITION:
It means cleaning and washing the external structure of the pelvic area and the genital area.
PURPOSE:
 To cleanse perineum
 To prevent infection
 To provide comfort
 To teach self-care and genital hygiene

EQUIPMENT:
 Flushing tray Cotton balls
 Flushing can
 Wash cloth
 Warm water
 Receptacle for soiled cotton balls
 Kidney basin
 Soap
 Bed pan

Assessment:
1. Assess client's perineal area and the extent of
soiling.
2. Identify the patient's capabilities.
Planning:
3. Wash hands.
4. Prepare all the equipment.
Implementation:
5. Identify the patient. Explain the procedure to
the patient.
6. Provide privacy
7. Place the patient in dorsal recumbent position.
8. Place a rubber sheet under the patient buttocks
and place client in the bed pan.
9. Expose perineal area and drape.
10. Wash hands and put on a clean-gloves
11. Perform the procedure as follows:
For Female patient:
12. Place the bath blanket down to the mid-thigh.
13. Flush perineal are with warm water.
14. Clean the vulva from the anterior portion
towards the anus then discard cotton balls
15. Gently separate the labia with gloves finger and
clean between the labia majora and labia
minora.
16. With labia separated, clean the clitoris urethral
meatus and vaginal orifice.
17. The thigh is cleansed using a stroke away from
the perineum.
18. Rinse and pat dry
For Male patient:
19. Place the bath blanket down to the mid-thigh.
20. Move the cotton balls from the tip of the penis
down its length towards the pubic area.
21. Hold shaft of penis and if uncircumcised, retract
the foreskin then pull back after.
22. Use a circular motion to clean the meatus and
glans penis in outward direction.
23. Rinse and pat dry.
24. Clean and rinse scrotum
25. Turn the patient on his side, wash and rinse
anal area.
26. Replace bath blanket with top sheet.
27. Clean all equipment and put them in proper
place.
28. Remove and discard gloves.
29. Wash hands.
Evaluation:
30. Evaluate patient for feeling of comfort.
31. Evaluate for unusual observation on the
perineal genital area.
Documentation:
32. Record the procedure done and pertinent
observation like redness, swelling on the area.
